椎板间开窗潜行减压治疗退行性腰椎管狭窄症(115例分析报告)

【摘要】 行椎板间开窗潜行减压治疗退行性腰椎管狭窄症115例,平均年龄54.2岁,病程平均3年9个月;全部病人都有腰痛和一侧或两侧下肢痛及麻木,均有间歇性跛行。除X线平片外,CT、CTM和/或MRI检查101例,椎管造影24例。其中伴椎间盘突出7例。狭窄段位于L4,549例,L5S125例,L3,43例,两个节段(L4,5和L5S1或L4,5和L3,4)26例,三个节段(L3,4,L4,5,L5S1)12例。术后复查CT/MRI 59例。结果84例获得随访,平均时间3年7个月,优51例,良27例,优良率91.66%。认为椎板间开窗潜行减压是治疗退行性腰椎管狭窄症的有效方法。该法简便、能维持后柱的稳定性。

【Abstract】 The results of lumbar spinal stenosis treated by undercutting the laminae of lumbar spine were reported in this article.The average age of 115 patients was 54.2 years, the average history was 3 years and 9 months.The main com-plaint was pain in back and in one or two legs, numbneas in the involved legs. They all had the symptom of neurogenic claudication. One hundred and one cases were examined with CH、CTM and/or MRI, 24 cases were examined with myelography in addition to the X - ray plate.Fifty seven cases complicated with protrusion of intervertebral disc.Forty nine cases occurred at the level of L4~5, 25 cases at the level of L5S1, and 3 cases at the level of L3-4, 26 cases occurred at two levels of L4~5 and L5S1 or L3~4 and L4~5.3 cases occurred at 3 levels (L3~4, L4~5 and L5S1) .Fifty nine cases were reexamined with CT/MRI after operations. Results showed that 84 cases were followed up and the average following -up was 3 years and 7 months.The rate of excellent and good result was 91.66% .Authors consider that decompression of spinal canal by undercutting the laminae through the interspace of laminae for the treatmeat of lumbar spinal stenosis was effective and easy and the stability of the spine was not endangered.
