慢性肾炎及中医证型与血中内皮素、一氧化氮关系探讨

【摘要】 为了解血浆内皮素(ET)与一氧化氮(NO)在慢性肾炎患者及中医证型中的变化,用放射免疫法测量血浆ET含量,用镉活化比色法测定血清NO含量。结果:123例慢性肾炎患者血ET显著高于正常人组(P< 0.01),血NO与正常人组无显著差异(P> 0.05),血ET含量在中医各证型中依次为正常人组< 肺肾气虚组< 脾肾阳虚组< 气阴两虚组< 肝肾阴虚组。血NO含量在中医各证型中依次为肺肾气虚组> 气阴两虚组> 正常人组> 肝肾阴虚组> 脾胃阳虚组。在肺肾气虚组和气阴两虚组中NO与ET无相关性(P> 0.05)。在脾肾阳虚组和肝肾阴虚组中ET和NO呈显著负相关(P< 0.05)。结论:慢性肾炎患者血中ET、NO平衡关系的破坏可能与慢性肾炎的发生发展有关,血浆ET含量的增加可以作为判断肾阴虚严重程度的一个客观指标,血清NO含量的变化可以反映出脏腑病损的严重程度

【Abstract】 To investigate the changes in circulating endothelin(ET) and nitric oxide(NO) of chronic glomerulonephritis and TCM syndromes of chronic glomerulonephritis,the level of plasma ET was measured using radioimmunoassay,while the level of serum NO was measured using cadmium reduction bar chromatographic analysis and colorimetry.Results:Compared with the control group,the plasma ET level was much higher in 123 patients with chronic glomerulonephritis(P<0.01),while the serum NO level did not show any significant difference between the two groups(P>0.05).The serum NO level was negatively correlated with the plasma ET level(P<0.01).The plasma ET level of different TCM syndromes increased according to the following sequence:the control group,the deficiency syndrome of lung and kidney Qi,the deficiency syndrome of spleen and kidney Yang,the deficiency syndrome of both Qi and Yin,the deficiency syndrome of liver and kidney Yin;while the serum NO level decreased according to the following sequence:the deficiency syndrome of lung and kidney Qi,the deficiency syndrome of both Qi and Yin,the control group,the deficiency syndrome of liver and kidney Yin,the deficiency syndrome of spleen and kidney Yang.There was no relationship between serum NO and plasma ET levels in the dificiency syndrome of the lung and kidney Qi,and in the deficiency syndrome of both Qi and Ying(P>0.05),The serum NO and plasma ET levels were negatively correlated in the deficiency syndrome of spleen and kidney Yang and the deficiency syndrome of liver and kidney Yin(P<0.05).Conclusion:The imbalance of plasma ET and serum NO may have relations with the occurrence and development of chronic nephritis.The plasma ET level can serve as an objective parameter to determine the degree of kidney Yin deficiency.The serum NO level can reflect the degree of Zang fu organ damage.
