可携带血氧仪对睡眠呼吸暂停综合征的诊断价值

【摘要】 目的 将可携带血氧仪与多导生理记录仪监测结果对比,探讨对睡眠呼吸暂停综合征(SAS)的初筛和确诊的临床应用价值。 方法 以睡眠呼吸紊乱指数(AHI)和最低血氧饱和度为标准将患者分为非SAS组与轻度、中度、重度SAS组,同时进行多导生理记录仪和血氧饱和度监测。 结果 AHI与氧减饱和指数(DI4)相关性好,诊断标准以DI4≥5 时,敏感性91.8% ,特异性81.3% ;DI4≥20 时,特异性98% ,敏感性69.4% 。 结论 选用血氧仪DI4 和SIT90指标对初筛或判断SAS患者夜间缺氧和病情严重程度具有一定价值。

【Abstract】 Determine the utility of a new portable nocturnal oximetry as a diagnostic tool for sleep apnea hypopnea syndrome(SAS) and compared with standard polysomnography(PSG) \ Methods\ A total of 98 patients were divided into five groups based on the apnea hypopnea index(AHI) and lowest SaO\-2(LSaO\-2,%) as the definition for SAS \ All patients underwent simultaneous all night recording of PSG and nocturnal oximetry \ Results\ The results showed that the notably correlation was found between AHI and decrease oxygen>4% index(DI\-4) \ Using a DI\-4≥5 as the definition,the sensitivity of the monitor in detecting SAS is 91 8%,the specificity is 81 3% \ When DI\-4≥20,the sensitivity is 69 4%, and the specificity is 98% \ Conclusion\ Using a DI\-4 and the time of SaO\-2<90%(SIT\-\{90\}) as the diagnostic standard,portable nocturnal oximetry can estimate the presence or absence of nocturnal breathing abnormalities with sufficient accuracy to be clinically useful in SAS \;
