宫内发育迟缓儿体格发育指标分析

【摘要】 目的 比较分析同期出生的宫内发育迟缓早产儿和足月儿体格发育指标的改变及其特点。 方法 测定并计算424 例早产小于胎龄儿、早产适于胎龄儿及足月小于胎龄儿出生时体重、身长及其Z 积分,并探讨其关系。 结果 各组IUGR 儿出生时体重和身长均明显低于同胎龄儿,死亡儿组更明显( P< 0 .01) 。Z 积分更能直观反映测量值代表的生长状态。体重Z 积分和身长Z 积分呈直线相关关系。直线斜率显示存活儿和死亡儿身长比体重均更易受到宫内生长迟缓的影响。 结论 IUGR 儿出生时体重与身长均明显落后,以身长落后更明显。

【Abstract】 Objective To investigate and compare the changes and characters of body proportionality in infants born with premature and/or intrauterine growth retardation(IUGR). Methods The cohort consisted of 424 infants from 3 000 newborns during 1995~1997,in which,there were53 preterm IUGR(deceased 11),263 preterm proportional infants(deceased 22) and 108 term IUGR(deceased 6).Birth weight and birth length and their Z-scores were calculated.Comparing and analyzing each groups with surviving and deceased infants respectively.Reduction of weight, length was analyzed using Z-scores based on Chinese Birth Standards. Results Preterm IUGR induced 25% deceased infants which was much higher than 8% in term IUGR infants.Both birth weight and birth length of IUGR infants was less than that of Chinese Birth Standards and much less in deceased infant group.Z-scores of weight and length were correlated in the surviving and deceased infants.Birth length was significantly more effected by IUGR than weight. Conclusion Z-scores allow us to pinpoint precisely any given measurement,noting progression or regression in relation to the median. Birth weight and birth length of IUGR infants was reduced and more significant in deceased infants and reduction of birth length was much more significant than that of birth weight.
