【摘要】 目的：研究高血压病（ｅｓｅｎｔｉａｌｈｙｐｅｒｔｅｎｓｉｏｎ，ＥＨ）病人胰岛素抵抗（ｉｎｓｕｌｉｎｒｅｓｉｓｔａｎｃｅ，ＩＳＲ）与纤溶酶原激活物抑制物１（ＰＡＩ１）的关系。方法：ＥＨ病人５９例，正常对照４６例。测定血浆ＰＡＩ１、ｔＰＡ（组织型纤溶酶原激活物）、空腹血糖（ＧＬＵ）、胰岛素（ＩＮＳ）、低密度脂蛋白胆固醇（ＬＤＬＣ）、高密度脂蛋白胆固醇（ＨＤＬＣ）、甘油三酯（ＴＧ）、胰岛素敏感指数（ＩＳＩ）、血尿酸（ＵＡ）、体重指数（ＢＭＩ）、腹围／臀围比（ＷＨＲ）等指标。结果：ＥＨ病人ＩＳＩ（用空腹血糖与胰岛素乘积的倒数表示）较正常对照组下降（－５．４±０．５ｖｓ－３９±０．８，Ｐ＜０．０１）。血浆ＰＡＩ１较对照组升高［（２９±８）ｍｇ·Ｌ－１ｖｓ（２４±５）ｍｇ·Ｌ－１，Ｐ＜０．０１］。在高血压组中单相关分析表明ＰＡＩ１与ＷＨＲ（ｒ＝０．３６９，Ｐ＜０．０１）、ＧＬＵ（ｒ＝０．５６３，Ｐ＜０．００１）正相关，与ＩＳＩ（ｒ＝－０．３０１，Ｐ＜０．０５），ＨＤＬＣ（ｒ＝－０．４１７，Ｐ＜０．０１）、ｔＰＡ（ｒ＝－０．４９９，Ｐ＜０．００１）负相关。以ＢＭＩ、ＷＨＲ、ＩＳＩ、ＨＤＬＣ、ＬＤＬ?更多还原

【Abstract】 Objective: To investigate the relationship between insulin resistance (ISR) and plasminogen activator inhibitor type 1(PAI 1) in essential hypertensive patients( EH). Methods:59 EH patients, and 46 controls were investigated. Body mass index(BMI),waist/hip ratio(WHR), plasma PAI 1 antigen,tissue type plasminogen activator(tPA), triglyceride( TG) , high density lipoprotein cholesterol( HDL C), low density lipoprotein cholesterol(LDL C), uric acid(UA), fasting glucose(GLU) and insulin(INS) levels were measured, and insulin sensitivity determined by insulin sensitive index(ISI), namely 1/(GLU×INS). Results: Compared with controls, in EH group, ISI was lower (-5.4±0.5 vs -3.9±0.8, P <0.01), and plasma PAI 1 antigen was higher [(29±8) vs (24±5) mg·L -1 , P <0.05].PAI 1 correlated significantly with insulin resistance,represented by ISI( r =-0.301, P <0.05). PAI 1 also correlated significantly with WHR( r =0.369, P <0.01),GLU( r =0.563, P <0.001),HDL C( r =-0.417, P <0.01), and tPA( r = -0.499 , P <0.001). Multiple regression analysis including indices of BMI,WHR,ISI,HDL C,LDL C, TG, UA,tPA,PAI 1, and INS disclosed ISI to account for 8.0% of variance of PAI 1antigen( P = 0.024 ). Conclusion:ISR and higher PAI 1 level were seen in EH patients. PAI 1 higher correlated with ISI, tPA, WHR, HDL C PAI 1 independently correlated with ISR.更多还原
