穴位针刺的功能性磁共振成像(fMRI)研究:基于坐标的荟萃分析

【摘要】 背景:功能性磁共振成像(f MRI)已被广泛用于研究不同穴位的大脑效应。然而,不一致的结果阻碍了我们对穴位确切大脑效应的理解。目的:对关于不同穴位针灸的f MRI研究进行荟萃分析,以评估现有研究中的针灸大脑图谱。方法:基于PROSPERO中的预注册协议(CRD42019119553),截至2024年8月9日搜索了大量数据库,没有语言限制。从显示针灸治疗前后信号差异显着的簇中提取峰值坐标。使用基于种子的d映射和主题图像置换(SDM-PSI)进行荟萃分析,这是一种新改进的荟萃分析方法。结果:共纳入82项研究(27项ST36;6项KI3;13项LI4;9项LR3;9项GB34;8项GB37;5项SP6;5项PC;5项ST36 vs假性ST36)。这项荟萃分析发现,每个穴位都有自己的大脑图谱。功能特征表明这些穴位主要与认知和感知有关。结论:我们的研究结果提供了八个穴位的大脑图谱,这不仅有助于更好地理解潜在的神经机制,也为未来的精准治疗提供了可能性。

【Abstract】 Background:Functional magnetic resonance imaging(f MRI) has been widely used to investigate brain effect of different acupoints.However,the inconsistent results have hindered our understanding of the exact brain effect of acupoints.Objective:To perform a meta-analysis of f MRI studies about acupuncture at different acupoints to assess brain atlas of acupuncture from available studies.Method:Based on a preregistered protocol in PROSPERO(CRD42019119553),a large set of databases were searched up to August 9,2024,with no language restrictions.Peak coordinates were extracted from clusters that showed significant signal differences between before and after acupuncture treatment.Meta-analysis was performed using seed-based d mapping with the permutation of subject images(SDM-PSI),a newly improved meta-analytic method.Results:Eighty-two studies(27 ST36;6KI3;13 LI4;9 LR3;9 GB34;8 GB37;5 SP6;5 PC;5 ST36 vs Sham ST36) were included.This meta-analysis found that each acupoint has its own brain altas.Functional characterizations showed that these acupoints were mainly associated with cognition and perception.Conclusion:Our results provide brain altas for eight acupoints,which not only offered a better understanding of the underlying neural mechanism,but also provided the possibility for future precision therapy.
