【摘要】 "调神七针法"和"膀胱五针法"是湖北省中医院针灸科韦丹主任传承和创新湖北中医大师李家康教授"补肾祛瘀,调和阴阳"学术思想,根据自身经验并结合科室多年临床用穴总结提出的针刺方法。临床实践中发现,尿失禁患者因疾病缠绵反复,病程日久,常伴有烦躁、焦虑甚至抑郁的情绪,《素问》有云:"用针之要,勿忘养神",从"调神"的角度出发,在治疗时注意调治患者心神,同时疏导患者情绪,故总结出"调神七针法"。"调神七针法"是把百会、玉枕、风池、太阳、内关、神门、三阴交七个腧穴有机地结合在一起,通过调督、补肾、养心,达到调神的目的。百会调理诸阳经及督脉之气血;玉枕、风池为黄鼎坚教授常治疗失眠之经验用穴,玉枕穴间接激发阳跷经气,以达调和阴阳、安神助眠之效;风池,治疗一切风邪疾病,通过针刺风池意在泄其邪气;太阳为经外奇穴,针刺提神醒脑、振奋精神;内关、神门,针之调和心经气血、养心安神;三阴交为脾经、肝经、肾经交汇处,调理三经之气血。"膀胱五针法"是在"补肾祛瘀,调和阴阳"治疗思想的基础上,结合针灸科治疗尿失禁临床用穴总结而来。膀胱五针法分别取穴中极、中极旁开1寸处(中极左右各1穴)、归来(双侧)共五针。中极为膀胱之募穴,针之可清利湿热、通利膀胱;中极旁开1寸处穴位是韦丹主任经验用穴;归来是足阳明胃经的穴位,意在疏导胃经气血,助膀胱气化。尿失禁其病位不仅在膀胱,同样与肾密切相关,其病机与肾虚,膀胱气化失司相关,临床上以肾气不固证型多见。前列腺增生属于"精癃"的范畴,其根本病机为肾气虚衰,其病理产物为瘀血、湿热、痰浊,三者互为因果,最终表现为肾虚为本,瘀血、湿热、痰浊为标之象。尿失禁、前列腺增生虽为不同疾病,但是在它们各自病程阶段均可出现肾气亏虚之证,表现出相应肾虚临床症状,且伴随一定程度的焦虑、失眠,由此考虑应用"调神七针法"联合"膀胱五针法"治疗以肾虚为基本病机的尿失禁、前列腺增生疾病。同时艾灸神阙、关元穴,借助艾条温通经脉以扶助正气、温补肾阳以助膀胱气化功能,达到治疗疾病的目的。更多还原

【Abstract】 "Mind-Regulating Seven-Needle Method" and "Bladder Five-Needle Method" are inherited and innovated by the director of acupuncture department Weidan of Hubei Hospital of Traditional Chinese Medicine,Professor Li Jia kang,a master of Traditional Chinese Medicine in Hubei Province,is the academic thought of "Tonifying Kidney and Removing Blood Stasis and Reconciling Yin and Yang".The acupuncture method is summarized according to his own experience and combined with the clinical acupoints used in the department for many years.In clinical practice,It is found that patients with urinary incontinence are often accompanied by irritability,anxiety and even depression because of repeated diseases and long course of disease.There is a saying in "plain question":"To use needles,do not forget to recuperate the mind",from the point of view of "Regulating the Mind",pay attention to regulating the mind of the patients during treatment,and at the same time dredge the mood of the patients,so summed up the "Seven Needles Method of Regulating the Mind".The "Mind-Regulating Seven-Needle Method" organically combines Baihui,Yuzhen,Fengchi,Taiyang,Neiguan,Shenmen and Sanyinjiao seven acupoints to achieve the purpose of regulating the mind by regulating the governor,tonifying the kidney and nourishing the heart.Baihui regulates the qi and blood of Zhuyang meridians and governor vessels;Yuzhen and Fengchi are the acupoints of Professor Huang Dingjian’s experience in the treatment of insomnia.Yuzhen points indirectly stimulate Yang stilts to achieve the effect of harmonizing yin and yang,calming the mind and helping sleep;Fengchi,treating all wind evil diseases,is intended to vent its evil qi by needling Fengchi;Taiyang is the strange acupoint of meridians,and acupuncture refreshes the brain and refreshes the spirit;Neiguan and Shenmen,acupuncture to reconcile the heart meridian qi and blood,soothe the mind and calm the mind.Sanyinjiao is the confluence of spleen meridian,liver meridian and kidney meridian to regulate the qi and blood of the three meridians."Bladder Five-Needle Method" is summarized on the basis of the treatment idea of "Tonifying Kidney and Removing Blood Stasis and Reconciling Yin and Yang",combined with acupuncture and moxibustion in the treatment of urinary incontinence.The bladder five-Needle Method was used to take a total of five needles at the middle pole,1 inch beside the middle pole(one point on the left and right side of the middle pole) and return(both sides).The acupuncture point of the middle extreme bladder can clear dampness and heat and clear the bladder;the acupoint 1 inch beside the middle pole is the empirical acupoint of Director Wei Dan;and the return is the acupoint of the stomach meridian of foot Yangming,which is intended to dredge the qi and blood of the stomach meridian and help the bladder gasify.The location of urinary incontinence is not only in the bladder,but also closely related to the kidney,and its pathogenesis is related to kidney deficiency and bladder gasification.Benign prostatic hyperplasia belongs to the category of "essence",and its fundamental pathogenesis is deficiency of kidney qi,and its pathological products are blood stasis,damp-heat and phlegm-turbid,which are cause and effect each other,and the final manifestation is kidney deficiency-based,blood stasis,damp-heat and phlegm-turbid as the standard.Although urinary incontinence and benign prostatic hyperplasia are different diseases,the syndrome of deficiency of kidney qi can appear in their respective stages of disease,showing the corresponding clinical symptoms of kidney deficiency,accompanied by a certain degree of anxiety and insomnia.Therefore,consider the application of "mind-regulating seven-needle method" combined with "bladder five-needle method" to treat urinary incontinence and benign prostatic hyperplasia with kidney deficiency as the basic pathogenesis.At the same time,moxibustion at Shenque and Guanyuan points,with the help of moxa to warm the meridians to help the vital qi,warm and tonify the kidney yang to help the bladder gasification function,to achieve the purpose of treating diseases.更多还原
