【摘要】 目的:介绍我院药物治疗管理(MTM)药学门诊的工作流程与成果,分析优点与不足,为其他药学门诊的建立提供参考。方法:介绍我院MTM药学门诊的建立过程、开设目的、服务人群、服务模式、团队构建、管理模式和学术交流方面的经验。统计门诊开设以来的就诊、复诊人次。统计分析前300例患者的一般情况、疾病情况、用药情况、药物治疗相关问题、药学服务内容和咨询问题种类。结果:我院MTM门诊共接诊425人次。37.4%的患者同时患有3种及以上慢性疾病,44%的患者用药数量为6-10种。药师共发现237名患者存在478个药物治疗相关问题,平均每人2.02个问题。有135名患者主动咨询了问题。结论:临床药师通过建立MTM药学门诊,参与患者慢病药物治疗管理,创新药学服务模式,促进药师角色转型。更多还原

【Abstract】 Objective: To introduce the working process and effects of pharmacist-managed clinic with medication therapy management mode in our hospital, analyzed the advantages and disadvantages, and provided reference for the establishment of other pharmacist-managed clinics. Methods: Introduced the experience of the establishment process, purpose, service population, service mode, team construction, management mode and academic exchange of MTM pharmacist-managed clinic. Count the number of visits and reviews since the clinic was opened. The general situation, disease status, medication status, medication-related problems, pharmaceutical service and consulting questions of the first 300 patients were collected and analyzed. Results: A total of 425 people were admitted to the MTM clinic. 37.4% of the patients had 3 or more chronic diseases at the same time, and 44% of the patients used 6 to10 kinds of drugs. The pharmacists found a total of 478 medication-related problems in 237 patients, with an average of 2.02 problems per person. 135 patients asked questions initiatively. Conclusion: Clinical pharmacists can participate in the management of chronic disease medications through the MTM pharmacist-managed clinics, innovate pharmaceutical service modes, and promote the transformation of pharmacists’ roles.更多还原
