针刺“环跳”和“委中”对坐骨神经损伤大鼠L4-L5脊神经节雪旺细胞、NCAM及CNTF表达的影响

【摘要】 目的:观察针刺"环跳"和"委中"对坐骨神经损伤大鼠L4-L5脊神经节神经细胞黏附因子(NCAM)、睫状神经营养因子(CNTF)表达的影响。方法:将80只SD大鼠随机分为假手术组、模型组、环跳组、委中组;每组各20只。采用坐骨神经钳夹损伤模型,两治疗组分别于造模后第8天予2Hz频率电针治疗,每次15min/天,治疗14天。检测大鼠坐骨神经功能指数(SFI),HE染色观察L4-L5节段脊神经节形态学变化,免疫组织化学法检测L4-L5节段脊神经节中NCAM、CNTF表达。结果:与假手术组相比,造模后各组SFI显著下降(P>0.05);HE染色显示神经纤维排列紊乱,雪旺细胞增多、肿胀,髓鞘崩解;NCAM、CNTF蛋白阳性表达减少(P<0.05);经电针治疗后,对比模型组和委中组,环跳组SFI数值显著升高(P<0.01);环跳组、委中组与模型组HE染色比较:神经纤维数量增多,排列紊乱程度减轻,雪旺细胞减少,并且上述病理改善程度环跳组优于委中组;与模型组对比,环跳组和委中组中NCAM、CNTF蛋白的阳性表达显著增加(P<0.05),且环跳组NCAM、CNTF蛋白阳性表达比委中组增加(P<0.05)。结论:电针针刺环跳和委中穴可促进坐骨神经损伤大鼠运动功能的恢复,增加NCAM、CNTF蛋白的分泌,修复受损坐骨神经、促进再生,并且环跳电针组优于委中电针组。

【Abstract】 Objective To observe the effect of different acupoints("Huantiao",Weizhong") of electroacupuncture(EA) on limb locomotion and the expression of Neural cell adhesion molecule,Ciliary neurotrophic factor in lumber(L)4—L5 of dorsal root ganglia in rats with sciatic nerve injury(SNI), so as to reveal its mechanisms underlying improvement of SNI.Methods A total of 48 SD rats were equally divided into sham control, model, "Huantiao" and Weizhong" groups. The SNI model was established by clamping the spinal nerve. EA intervention(2 Hz, 1 mA), starting on the 8 th day after modeling, was applied to "Huantiao" or"Weizhong" on the injured side for 15 min, once daily for 14 consecutive days. The sciatic function index(SFI) was calculated to assess the injured hindlimb recovery with reference to BAIN’s and colleagues’ methods. Histopathological changes of the sciatic nerve were displayed by H.E. staining. The expressions of "NCAM", "CNTF" in the sciatic nerve tissue were detected.Results After modeling, the SFI level on day 8 was significantly decreased in the model group(P<0.01), and no significant differences were found among the model, "Huantiao" and"Weizhong" groups before the EA intervention(P>0.05). Following the treatment(at the 22 nd day), the SFI values of both "Huantiao" and "Weizhong" groups were significantly increased(P<0.01), suggesting an improvement of the limb motor function, and the SFI of the "Huantiao"group was notably higher than that of the "Weizhong" group(P<0.01). In comparison with the sham control group, the expression levels of "NCAM", "CNTF" in the sciatic nerve were significantly down-regulated(P<0.05). Following EA intervention, the increased expression levels of "NCAM", "CNTF" were significantly up-regulated in both "Huantiao" and"Weizhong"groups(P<0.05), and the therapeutic effect of "Huantiao" was markedly superior to that of "Weizhong"in up-regulating the expression of "NCAM", "CNTF"(P<0.05). Increase of Schwann cells in number and disintegration of the axons and myelin sheath in the model group,which was relatively milder in both "Huantiao" and "Weizhong" groups, particularly in the"Huantiao"group. Conclusion EA of GB30 and BL40 can promote the recovery of hindlimb motor function in SNI rats, and facilitating the repair of the damaged sciatic nerve. "Huantiao" is better than "Weizhong" in the therapeutic effect.
