Taylor空间支架与单臂外固定支架治疗胫腓骨开放骨折的临床分析

【摘要】 目的探讨比较Taylor空间支架与单臂外固定支架治疗胫腓骨开放骨折的临床疗效。方法回顾性分析2015年2月至2018年4月于天津医院矫形一病区治疗的胫腓骨开放骨折患者60例,根据手术方法分为Taylor空间支架治疗组(30例)与单臂外固定支架治疗组(30例),两组患者性别、年龄、侧别、致伤原因、骨折分型及受伤至手术时间等一般资料差异均无统计学意义(P>0.05),具有可比性。比较两组患者的手术时间、手术费用、骨折愈合时间、外固定支架拆除时间、术后并发症发生率,患侧肢体胫股角优良率及拆除外固定支架6个月后Johner-Wruhs胫骨干骨折治疗最终效果评分优良率。结果与单臂外固定支架治疗组相比,Taylor空间支架治疗组骨折愈合时间以及外固定架拆除时间明显缩短(P<0.05),术后并发症发生率较低(P<0.05),患侧肢体胫股角优良率与Johner-Wruhs胫骨干骨折治疗最终效果评分优良率较高(P<0.05)。结论应用Taylor空间支架治疗胫腓骨开放性骨折临床疗效显著,相比单臂外固定架骨折愈合时间短、术后并发症少、患肢功能恢复好,可在临床中更多推广应用。
【Abstract】 Objective: To investigate the clinical analysis of Taylor space frame and unilateral external fixation in the treatment of open tibiofibular fractures. Methods: To have a retrospective analysis of 60 patients with open tibiofibular fractures treated by surgery in orthopaedic 1 area of Tianjin hospital from February 2015 to April 2018 was conducted. According to the surgical method, it was divided into Taylor space fixator frame group(30 cases) and unilateral external fixation group(30 cases). The differences between the two groups were not statistically significant(P>0.05) and could be compared with the general data of gender, age, side type, cause of injury, fracture type and time from injury to surgery. The fracture healing time, external fixation removal time, postoperative complication rate of the two groups,Johner-wruhs tibial shaft fracture of the affected limb 6 months after the removal of external fixator and tibia femoral Angle of the affected limb were compared. Results:Compared with the unilateral external fixator treatment group, the fracture healing time and the removal time of external fixator in the Taylor space frame treatment group were significantly shortened(P<0.05), and the incidence of postoperative complications was relatively low(P<0.05). The excellent rates of the affected limb in tibia femoral angle and final outcome score by Johner-wruhs tibial shaft fracture treatment final outcome score were higher(P<0.05).Conclusion Taylor space fixation has a significant clinical effect in the treatment of open tibiofibular fractures, and it has a shorter healing time, less postoperative complications, and better joint function than unilateral external fixator fracture, which can be more widely used in clinical practice.
