【摘要】 目的本文旨在探讨老年颅脑疾病患者合并非腹源性肠梗阻的临床诊断与治疗。方法回顾性分析2019-09-01至2019-10-25新疆维吾尔自治区人民医院神经外科住院期间发生非腹源性肠梗阻患者的临床资料,对出现肠梗阻的临床表现、原因、分类及治疗等方面进行分析。结果同期共收治315例患者,≥55岁146例(46.4%);本组共收集7例患者,占总患者2.2%,占≥55岁患者4.8%;其中,颅脑疾病分别为4例颅内肿瘤(均在全麻下接受手术治疗),2例颅脑外伤,1例高血压脑出血;非腹源性肠梗阻分别为5例不完全性动力性肠梗阻,1例完全性机械性肠梗阻,1例不完全性血运性肠梗阻;其中死亡1例,肠造瘘1例,5例治愈。结论老年颅脑疾病患者最常合并非腹源性肠梗阻类型为动力性肠梗阻,动力性肠梗阻也可转变为机械性肠梗阻,血运性肠梗阻一旦诊断明确,立即手术治疗。掌握肠梗阻的临床表现、原因、分类及治疗可指导临床医生尽早发现及早期干预肠梗阻发生,进而减少继发性脑和其它脏器的损伤,提高颅脑疾病的救治率。更多还原

【Abstract】 Objective This article aims to investigate the clinical diagnosis and treatment of non-primary gastrointestinal ileus in elderly patients associated with craniocerebral disease.Methods Clinical data of patients with non-primary gastrointestinal ileus were analyzed retrospectively who were admitted to department of neurosurgery,people’s hospital of xinjiang uygur autonomous region during their stay from September 01,2019 to October 25,2019.The clinical manifestations,causes,classification and treatment of intestinal obstruction were analyzed.Results During the same period,a total of 315 patients were admitted,including 146 patients aged ≥55 years(46.4%).A total of 7 patients were collected in this group,it accounted for 2.2% of the total patients and 4.8% of patients ≥55 years old.Craniocerebral diseases respectively were 4 cases of intracranial tumors(all of which received surgical treatment under general anesthesia),2 cases of craniocerebral trauma,and 1 case of hypertensive intracerebral hemorrhage.Non-primary gastrointestinal ileus respectively were 5 cases of incomplete dynamic ileus,1 case of complete mechanical ileus,and 1 case of incomplete vascular ileus.Among them,1 died,1 enterostomy,and 5 were cured.Condusions Dynamic ileus is the most common type of craniocerebral disease associated with non-primary gastrointestinal ileus in elderly patients.Dynamic ileus can also change to mechanical ileus.Once the diagnosis of vascular ileus is clear,surgical treatment should be performed immediately.Understanding the clinical manifestations,causes,classification and treatment of intestinal obstruction can guide clinicians to detect and intervene the occurrence of intestinal obstruction as early as possible,so as to reduce secondary brain and other organ injuries and improve the treatment rate of craniocerebral diseases.更多还原
