口腔微生物群落在慢性牙周炎与慢性阻塞性肺疾病(COPD)相关性机制中作用的研究

【摘要】 目的:流行病学研究发现慢性牙周炎与慢性阻塞性肺疾病(COPD)紧密相关,是其发病的危险因素,而口腔菌群可能是牙周炎影响COPD病理机制的关键通道。在本研究中,我们检测并分析唾液微生物菌的变化,评估它们与牙周炎和COPD的潜在联系。方法:我们通过纳入排除标准收集COPD伴慢性牙周炎患者21人,单纯慢性牙周炎患者36人和健康人群14人,使用16SrDNA高通量测序对受试人群唾液及菌斑微生物进行检测,对其口腔微生物细菌群类的定植情况及数量的多样性改变进行检测分析,筛查阳性微生物.结果:COPD患者的唾液及菌斑微生物丰度和多样性明显增高。受试样本中占比最大的菌分别是链球菌属(Streptococcus),普雷沃氏菌属(Prevotella),奈瑟菌属(Neisseria),罗氏菌属(Rothia),梭杆菌属(Fusobacterium)。健康对照组与牙周炎伴COPD组和单纯慢性牙周炎组的口腔唾液微生物群的组间比较具有统计显著差异(P<0.05)。在三组受试人群中,与单纯慢性牙周炎组和健康对照组比较,牙周炎伴COPD组中的韦荣氏菌、罗氏菌和放线菌显著升高,而单纯慢性牙周炎组中的检测到的这三种菌也明显高于健康对照组,差异具有统计学意义;而梭杆菌则有相反的趋势,在牙周炎伴COPD组中检出率最低。结论:口腔微生物菌群的变化与COPD和慢性牙周炎具有相关性。慢性牙周炎影响COPD的病理机制可能与口腔唾液微生物菌群中的一些阳性微生物显著差异有关,这对COPD风险评估、早期诊断、早期治疗具有现实意义。
【Abstract】 Objective:The oral microbiota plays a key role in the initial colonization by pathogens and the chronic inflammatory reaction of the host,with possible relevance to the connection between periodontitis and chronic respiratory disease(COPD).In the present study,we measured variations in the salivary microbiota and evaluated their potential associations with periodontitis and COPD.Methods:We investigated the salivary microbiota of patients with COPD and periodontitis(n=21) compared with that in patients with periodontitis alone(n=36) and with healthy controls(n=14),using pyrosequencing of PCR-amplified 16 s rRNA genes.Results:Bacterial richness and diversity were significantly higher in patients suffering from COPD,and the bacteria family Lachnospiraceae was frequently observed only among patients with COPD and periodontitis.Veillonella,Rothia,Actinomyces,and Fusobacterium were the core bacterial genera that showed significant differences among patients with coincident COPD and periodontitis,patients with periodontitis alone,and health controls(Kruskal-Wallis test,p <0.05).Additionally,Veillonella,Rothia,and Actinomyces were observed more frequently in the oral microbiota of patients with periodontitis and much more frequently in patients with both COPD and periodontitis,compared with that in the healthy controls.Conclusion:The data indicate that variations in patients’ salivary microbiota may be associated with COPD and periodontitis.The possible association between COPD and periodontitis may be related to significant differences certain residents of the oral saliva microbiota,which may be aninformative source to identify potential risk factors that contribute to the pathogenesis of COPD.
