骨关节结核患者麻醉药物使用的临床评价

【摘要】 目的探讨骨关节结核患者使用麻醉药物丙泊酚和异氟烷对炎性作用的影响。方法骨关节结核病灶清除术患者229例,根据性别随机分成丙泊酚组、异氟烷组,丙泊酚组患者使用麻醉药物丙泊酚3~6μg/mL,异氟烷组患者使用麻醉药物1%~2%异氟烷。两组患者分别于麻醉前即刻、手术后即刻、手术后12小时、手术后24小时各抽取静脉血5mL,测定血清白细胞介素1(IL-1)、肿瘤坏死因子α(TNF-α)、细胞黏附分子1(ICAM-1)的含量。结果丙泊酚组骨关节结核患者与异氟烷组相比,麻醉前即刻和手术后即刻血清IL-1、TNF-α、ICAM-1含量差别均无统计学意义;手术后12小时和术后24小时,丙泊酚组IL-1、TNF-α、ICAM-1含量均较异氟烷组降低(均P<0.05);2组患者血清ICAM-1、IL-1、TNF-α含量术后均呈上升趋势,各指标各检测点间差异均有统计学意义(均P<0.01)。结论麻醉药物丙泊酚比异氟烷可更好地减弱骨关节结核患者的炎性作用。

【Abstract】 Objective To investigate the effect of propofol and isoflurane on the inflammation in patients with osteoarticular tuberculosis. Methods Two hundred and twenty nine patients undergoing focal cleaning of osteoarticular tuberculosis were divided randomly into two groups, propofol group and isoflurane group. The former group patients were given propofol 3~6μg/mL, and the latter group patients received isoflurane 1% ~2%. The levels of IL-1, TNF-α, and ICAM-1 were recorded before anesthesia, 0 h, 12 h, and 24 h after operation. Results The serum levels of IL-1, TNF-α, and ICAM-1 were significantly higher in isoflurane group than those in propofol group at 12 h and 24 h after operation(all P<0.05). The serum levels of IL-1, TNF-α, and ICAM-1 were significantly increased at 0 h, 12 h, and 24 h after operation compared with those before anesthesia in both groups(all P<0.01). Conclusion The effect of propofol was better than isoflurane in inhibiting inflammatory reaction in patients with osteoarticular tuberculosis.
