右心声学造影及经颅多普勒超声对卵圆孔未闭诊断价值分析

【摘要】 目的对比右心声学照影(cTTE)与经颅多普勒发泡试验(cTCD)对卵圆孔未闭右向左分流的诊断价值分析。方法选临床确诊偏头痛患者112例,分别进行右心声学造影及经颅发泡试验检查,常规进行经胸心脏彩超检查(TTE),了解心脏结构、功能,之后进行右心声学照影及经颅TCD发泡试验。结果 cTTE发现左心房出现气泡影像65例,阳性率为58%;cTCD阳性患者48例,阳性率为42.8。偏头痛患者cTTE阳性率高于cTCD,有统计学意义(P<0.05)。结论 cTTE及cTCD试验结果提示cTTE更能准确而全面地诊断卵圆孔未闭右向左分流的存在,cTTE试验方法简便易行,敏感性高于cTCD,值得临床推广。

【Abstract】 Objective Application of contrast transcranial doppler(cTCD) and contrast transthoracic echocardiogaphy(cTTE)on patients with migraine headachediscuss the etiology ofmigraine and evaluatethe advantage of the two check methods.Methods We studied 195 people.All underwent cTCD-cTTE and echocardiography.Of these 195 people,112 patients were diagnosed migraine by clinicians,normal control group include 83 people.cTCD and cTTE were carried out on the foundation of understood the patients heart structure and function by echocardiography.Results In experiment group,we found bubble image appeared in left atrium in 65 cases by cTTE,the positive rate was 58%;48 patients with cTCD positive,the positive rate was 47.3%.In 85 control group,we found bubble image appeared in left atrium in 20 cases by cTTE,the positive rate was 24.1%;15 patients with cTCD positive,the positive rate was 18.1%.We found the rate of right-left-shunt in experiment group was much more than the normal control group by cTCD and cTTE.The positiverate of cTTEwas higher than cTCD.Conclusions The results of cTTE and cTCD indicated the cause of migraine may be associated with venous system within certain substances,cTTE was more simply、 easly、 sensitivity thancTCD,deserved the clinical expansion
