含气性和非含气性肝脓肿超声介入治疗的比较

【摘要】 目的比较产气性和非产气性肝脓肿介入性超声治疗的方法和效果。方法回顾性分析136例肝脓肿的超声介入治疗经验。按照脓肿有无含气分为含气性(12例)和非含气性(124例)肝脓肿,比较两组病人的临床特征、实验室和影像学检查结果以及超声介入治疗效果。结果(1)含气性化脓性肝脓肿(GFPLA)临床表现、实验室和影像学损害结果更严重,脓腔更大且更常合并糖尿病(P<0.01或0.05)。(2)与非含气肝脓肿相比,两组治疗效果相当,但GFPLA更常采用置管引流,治疗时间更长(P<0.05)。结论采用超声引导经皮置管引流,配合全身严格的抗生素治疗是GFPLA常用和有效的治疗方法。

【Abstract】 Objective To compare the clinical treatment results of interventional ultrasound between gas-forming and non-gas-forming pyogenic liver abscess.Methods136 patients with clinical diagnosis of pyogenic liver abscess(PLA) were enrolled in the study.PLAs were divided into two groups:gas-forming(12 cases) and non-gas-forming PLA(124 cases).The clinical features,laboratory findings,imaging outcomes and interventional ultrasound results were recorded and compared.Results 1.The gas-forming PLA had severer clinical features,laboratory and imaging findings,and the size of abscess was usually bigger.The incidence of concurrent diabetes mellitus was higher(p <0.05 or 0.01).2.To compare with non-gas-forming PLA,although the interventional ultrasound treatment results were similar between the two groups,treatment duration was longer and more used ultrasound-guided catheter drainage in gas-forming PLA group(p <0.05).Conclusions The ultrasound-guided percutaneous catheter can be a safe and effective method for treatment of gas-forming PLA on the basis of vigorous antibiotic therapy.
