【摘要】 目的探讨动态平板透视的狭缝全景摄影技术(SLOT)双下肢全长摄影在膝关节置换术的临床应用价值。方法回顾性分析2013年11月份至2017年3月份89例膝关节置换患者,共99例膝关节进行了全膝关节置换术,每位患者在手术前后分别使用动态平板透视摄影系统的狭缝全景技术进行负重位的双下肢全长摄影,采用0-2分的评分方式对图像质量进行评估,0分图像质量差,1分图像质量中等,2分者为图像质量优良,并在图像质量优良的图片上通过后处理软件对患者的手术膝进行术前和术后下肢力线及胫股角进行测量,对术前进行全面评估,同时通过对比分析评价手术效果。结果 89例患者双下肢负重位全长摄影全部一次成功,所有患者图像评分均为2分,质量优良,合并膝内翻畸形患者17例,膝外翻畸形患者8例,无合并内外翻畸形者64例,89例患者全膝关节置换术手术效果优良,25例膝内外翻畸形得到了良好的校正。结论使用狭缝全景摄影技术对双下肢全长进行成像,其成功率高、检查时间短、辐射剂量低,且图像拼接操作简便,质量优良,满足临床诊断需求,对临床术前手术方案制定及术后评估具有重要应用价值。更多还原

【Abstract】 Objective To investigate the clinical application value of total knee arthroplasty of SLOT of Dynamic flat-panel X-ray photography system in whole lower extremities.Methods From 2013 November to 2017 March,a retrospective analysis of 89 cases of total knee arthroplasty patients,a total of 99 knee joints,all of them received the radiography of whole lower extremities with standing position by SLOT in dynamic flat-panel X-ray photography system pre and post operation respectively.Using 0-2 scores to evaluate the image quality,0 score representatives poor image quality,1 score representatives medium image quality,and good image quality is counted 3 score.The lower limb alignm and femorotibial angle of patients’ surgery knee are measured with the postprocessing software in the good quality images,and all knees are evaluated completely before operation,meanwhile,the surgery effect is evaluated by contrastively analysed X-ray findings of the pre and post operations.Results All 89 patients with Genu varum(n=17),genu valgum(n=8) and no Genu varum or valgum(n=64) photography success once time.The score of all patients’ images is 2 respectively,which display good image quality.The surgery effect in 89 patients is good respectively,and 25 patients of genu varum or valgum has been corrected well.Conclusion SLOT Radiography of whole lower extremities checks high success rate,fast,low radiation dose,and has simple image stitching operation,images have excellent quality satisfied the needs of clinical diagnosis,and has important application value in preoperative planning and postoperative evaluation.更多还原
