2014-2016年广东和广西部分地区宋内志贺菌分离株病原学特征分析

【摘要】 目的了解2014-2016年广东省和广西壮族自治区部分地区宋内志贺菌暴发分离株及散发分离株的病原学特征。方法对2014-2016年广东省和广西壮族自治区柳州市分离的14株宋内志贺菌暴发菌株和6株散发菌株进行抗生素敏感性试验和脉冲场凝胶电泳(PFGE)分析,并选择其中6株代表菌株进行全基因组测序,与NCBI上获取的51株国内外宋内志贺菌的基因组进行遗传进化分析。结果抗生素敏感性检测发现,试验菌株对氨苄西林、四环素、庆大霉素、复方新诺明和萘啶酸有较高的耐药性,而对阿奇霉素、氯霉素和亚胺培南完全敏感。PFGE分子分型显示,不同地区不同来源的分离株之间PFGE指纹图谱具有很高的相似度(93.2%),而基于全基因组的遗传进化分析显示,广东省和广西柳州的宋内志贺菌分离株同处在一个进化分支上,与来自韩国的菌株亲缘关系最为接近。结论广东省和广西壮族自治区部分地区分离的宋内志贺菌对常用抗生素具有较高的耐药性,对阿奇霉素、氯霉素和亚胺培南有较高的敏感性。试验菌株之间具有相似的PFGE指纹图谱,遗传进化关系相近,需要继续加强本地区宋内志贺菌的病原学特征监测,以有效防控其引起的暴发流行。
【Abstract】 Objective To investigated the etiologic characteristics of Shigella sonneistrains causing outbreaks and sporadic cases in some regions of Guangdong and Guangxi province of China, 2014-2016. Methods 14 Shigella sonnei strains isolated from outbreaks and 6 strains from sporadic cases from Guangdong province and Guangxi Liuzhou city were tested for antimicrobial resistance and analyzed by pulsed-field gel electrophoresis(PFGE). Six typical strains were even selected for whole genome sequencing typing and compared with 51 strains isolated international available from NCBI genome database. Results The antibiotic resistance test indicated the isolates had high resistance rate to ampicillin, tetracycline, gentamicin, Trimethoprim/sulfamethoxazole and nalidixic acid, while quite sensitive to azithromycin, Chloromycetin and imipenem. PFGE showed high similarity(93.2%) between the strains isolated from different regions and types. The whole genome sequencing analysis also revealed all the typical strains clustered into a same evolution branch, closer to some strains from Korea. Conclusions The Shigella sonnei strains isolated from some regions of Guangdong and Guangxi province of China showed high resistance to commonly used antibiotics, while quite sensitivie to azithromycin, chloramphenicol and imipenem. Isolates in this study also showed similar PFGE patterns and close phylogenic evolution. It is necessary to continuously survey the etiologic characteristics of Shigella sonneifor control the outbreak in this region due to this pathogen.
