硫必利单用与联合脑电生物反馈治疗抽动障碍临床疗效对比研究

【摘要】 目的:探讨硫必利联合脑电生物反馈治疗抽动障碍疗效是否优于单用硫必利。方法:对符合ICD-10诊断标准的80例慢性运动性抽动或发声抽动障碍和Tourette氏综合征的患儿随机分为两组,分别使用硫必利治疗(药物治疗组,43例)、硫必利联合脑电生物反馈治疗(联合治疗组,37例),在治疗第0、4、12周采用耶鲁抽动症整体严重度量表(YGTSS)、Achenbaci儿童行为量表(CBCL)评分,比较两组间的抽动症状及社会能力改善。结果:治疗4W时药物组26例有效(60.5%),6例显效(14.0%);联合组20例有效(54.1%),12例显效(32.4%龃间无显著差异(P=0.10)。治疗12W时药物组29例有效(67.4%),8例显效(18.6%);联合组17例有效(45.9%),17例显效(45.9%),联合组疗效优于药物组(P=0.031)。治疗4W时两组的CBCL社会能力总分与基线水平比较均无明显差异,治疗1 2W时社会能力总分与治疗4周及基线水平比较均提高。结论:联合组短期疗效与药物单用相当,中期疗效优于药物单用;社会能力改善发生在抽动症状改善之后。
【Abstract】 Object:To explore whether tiapride combined with EEG BiofeedbacK training be more effective than tiapride in treatment of tic disorders.Methods:80 individuals with chronic tic disorder or Tourette’s syndrome were randomly assigned into two groups to be treated with tiapride or combined treatmentrespectively.We assessed the effect at 4 and 12 weeks later by using Yale Global Tic Severity Scale and Achenbach Child Behavior Checklist.Results:After 4 weeks treating,effective rate was 60.5%and significant effective rate was 14.0%in tiapride group,which were 54.1%and 32.4%in combined group and there was no significant statistical between-group difference(p=0.10).After 12 weeks treating,effective rate and significant effective rate were 67.4%and 18.6%in tiapride group and were 45.9%and 45.9%in combined group with significant between-group difference(p=0.031).ln both groups,the social competence scores of CBCL have no significant statistical difference between the baseline and 4-weeks-later.while scores of 12-weeks-later were significantly higher than that of baseline and 4 weeks.Conclusions:In treatment of tic disorders,tiapride combined with EEG Biofeedback training was more effective than tiapride after 12 weeks.The improvement of social competence happened after the alleviation of tic symptoms.
