【摘要】 目的探究艰难梭菌感染对结直肠癌患者营养及代谢的影响。方法留取205例肠癌患者术前粪便样本,检测有无艰难梭菌感染,记录术前营养、代谢及肿瘤病理指标。结果 T4(肿瘤侵出浆膜)及OB阳性的结直肠癌患者中,艰难梭菌感染者的白蛋白和总蛋白均较未感染者显著降低(P<0.05);N2(淋巴结转移超过3枚)感染者中总蛋白较未感染者显著降低(P<0.05);BMI(体质指数)≥24及男性感染者的游离脂肪酸水平较未感染者显著下降,且在年龄≥60岁的患者中更加显著(P<0.05)。结论艰难梭菌感染的T4、N2及OB阳性的结直肠癌患者易引起白蛋白、总蛋白下降;体重超重、男性的老年感染患者更易出现脂肪代谢紊乱;对这类患者的营养及代谢状况改变应引起充分重视,并作进一步的深入研究。更多还原

【Abstract】 objective To evaluate the effect of Clostridium difficile infection(CDI) on nutrition and metabolism of patients with colorectal cancer(CRC).Methods Stool samples were collected from 205 preoperative CRC patients who were scheduled to accept radical cancer resection.CDI status were detected.Laboratory and pathology findings were collected.Resultes T4(tumour penetrates the serosa) or OB positive CRC patients with CDI had significantly lower serum levels of albumin,total protein(P<0.05);N2(lymph nodes metastasis more than 3)CRC patients with CDI had significantly lower serum levels of total protein(P<0.05);Body Mass Index(BMI) ≥ 24 and male patients had lower serum levels of free fatty acid,especially in CRC patients over 60 years old(P<0.05).Conclusion CDI can decrease the serum levels of albumin,total protein and free fatty acid of T4 or OB positive CRC patients.Dyslipidiaemia can occure in CRC patients with CDI who were over 60 years old in BMI ≥ 24 or male population.More attedtion should be paid on the nutrition and metabolism of these people,and further studies are needed to reveal the mechanisms.更多还原
