【摘要】 目的通过GeneXpert MTB/RIF系统对经过规范治疗2月末痰菌仍阳性的肺结核患者进行检测,结合痰培养药敏及后期治疗情况,分析该检测对经过规范治疗疗效不佳患者的检测价值和预警价值。方法:随机选取实验组(治疗2月末痰菌阳性)、实验对照组(治疗2月末痰菌阴性)。检测价值分析:予以GeneXpert MTB/RIF系统对痰液进行痰找结核杆菌、利福平耐药基因检测,同时予以痰培养药敏试验,应用卡方检验分析。预警价值分析:治疗至5月末时,实验组的疗效情况与2月末的GeneXpert MTB/RIF系统的预测结果进行卡方检验分析。结果:GeneXpert MTB/RIF系统在实验组的结核杆菌检出率达到96.2%,耐利福平杆菌的检出率达到94.3%。对培养阳性,涂片阳性的结核杆菌检出率为96.2%,培养阳性,涂片阴性的结核杆菌检出率为50.0%。GeneXpert MTB/RIF系统的预测疗效与实际临床治疗效果没有统计学差异。结论:GeneXpert MTB/RIF系统对临床治疗不佳肺结核具有重要的检测价值和预警价值。更多还原

【Abstract】 Objective:Analysis the forecast value and test value of GeneXpert MTB/RIF at the Patients with poor therapeutic effects through the detection of pulmonary tuberculosis at treatment of the end of 2 months by GeneXpert MTB/RIF,compare with the mycobacterium tuberculosis culture,drug sensitivity test and treatment effectiveness evaluation.Methods:We selected experimental group(smear positive at the end of 2 months),control group(Smear negative at the end of 2 months) and normal group.Test value analysis:Use GeneXpert MTB/RIF to detect the mycobacterium tuberculosis,Rifampin(RFP) resistant gene compare with the mycobacterium tuberculosis culture,drug sensitivity test by compare with the mycobacterium tuberculosis,drug sensitivity test by chi-square.Forecast value:Treatment to the end of 5 months.Analysis between the treatment effect and forecast effect by GeneXpert MTB/RIF.Results:In experimental group,the positive rate of mycobacterium tuberculosis was 96.2%,the positive rate of Rifampin(RFP) resistant gene was 94.3%.The rate of mycobacterium tuberculosis was 96.2%to positive culture and smear positive by GeneXpert MTB/RIF.The rate of mycobacterium tuberculosis was 50.0%to positive culture and Smear negative by GeneXpert MTB/RIF.the study showed no statistical significance between the treatment effect and forecast effect by GeneXpert MTB/RIF.Conclusions:There are important forecast value and test value of GeneXpert MTB/RIF to the poor therapeutic effects patients.更多还原
