【摘要】 目的探讨有限内固定配合外固定架治疗复杂Pilon骨折的微创手术方法和临床疗效。方法病例来源于甘肃省中医院2011年10月~2014年09月收治的复杂Pilon骨折患者,其中观察组26例,采用有限内固定配合外固定架技术治疗;对照组为同期住院治疗的26例复杂Pilon骨折患者,采用切开复位内固定治疗。随访6~18个月,分析两组患者相关临床资料,比较两组患者踝关节功能、骨折愈合时间、住院时间及并发症发生率。结果观察组和对照组踝关节功能评定优良率分别为24例(92.3%)、19例(73%),差异具有统计学意义(P<0.05),观察两组患者X线片,观察组骨折愈合时间为(17.22±1.91)周,对照组骨折愈合时间为(19.90±2.67)周,观察组显著优于对照组(P<0.05);观察组并发症发生率为2例(7.7%),观察组为6例(23.1%),差别有统计学意义(P<0.05)。观察组术后住院时间小于对照组,差异具有统计学意义(P<0.05)。结论有限内固定配合外固定架治疗复杂Pilon骨折具有提高关节功能优良率,并且较传统内固定损伤小,缩短骨折愈合时间,有效减少住院时间的优点,是治疗复杂Pilon骨折的理想治疗手段。更多还原

【Abstract】 Objective Resarch the efficacy of external fixator and limited internal fixation for the treatment of complicated Pilon fractures of minimally invasive surgical method and clinical curative effect.Methods Select complex Pilon fracture patients where come from GanSu Hospita of TCM bettwen October 2011 to sep 2014,the observation group 26 cases,use limited internal fixation with external fixator technical treatment;Control group for the same period in the hospital treatment of 26 patients with fracture of complex Pilon fracture patients,use anatomic plate fixation treatment.All of 52 patients were followed up for 6~18 months,analyzed two groups of patients with relevant clinical data,compared two groups of patients with ankle function,fracture healing time,length of hospital stay,and complications.Results Ankle function rate observation group and control group respectively for 24 cases(92.3%),19 cases(73%),statistically significant difference(P<0.05),observe two groups of patients with X-ray slice,fracture healing time was(17.22±-1.91),fracture healing time of(19.90±2.67) for weeks,observation group was significantly better than the control group(P<0.05);The complication rate of observation group of 4 cases(15.38%),observation group for 5 cases(19.23%),statistically significant difference(P<0.05).Observation group of postoperative hospital stay is less than the control group,the difference statistically significant(P<0.05).Conclusion Limited internal fixation combined with external fixator for the treatment of complicated Pilon fractures is raising the rate of good joint function,and less damage than traditional internal fixation,shorten fracture healing time,effectively reduce the length of time,the advantages of is an ideal treatment for the treatment of complicated Pilon fractures.更多还原
