【摘要】 目的回顾性分析中药联合经髋关节外科脱位入路打压植骨支撑术治疗中青年ARCOⅢ期股骨头坏死的临床疗效。方法回顾性分析中青年ARCOⅢ期股骨头缺血性坏死患者,共22例(22髋),创伤性坏死2例,酒精性坏死16例,激素性坏死4例,所有患者均采用仙灵骨葆胶囊和磷酸川芎嗪片口服联合经髋关节外科脱位入路打压植骨支撑术治疗;以Harris评分评价关节功能,以股骨头塌陷超过4mm为终点事件,以股骨头生存率作为疗效评价指标。结果所有患者术后均未发生感染和血栓,一例患者术后出现腓总神经损伤症状,经治疗,术后10个月踝关节背伸肌力恢复至3级;术后随访6-18个月(平均11.8个月),Harris评分从术前平均(70.3±4.6)提高到末次随访平均(91.0±6.3)分;末次随访时股骨头生存率为95.5%。结论中药联合经髋关节外科脱位入路打压植骨支撑术治疗中青年ARCOⅢ期股骨头坏死短期临床疗效确切,远期疗效有待进一步观察。更多还原

【Abstract】 Objective:To evaluate the effect of impacting TCM combined with Bone Graft through Dislocation of Hip Joint Surgical Approach Treating ARCOⅢ Stage ONFH in young and middle-aged with retrospective study.Methods:22(22 hips) ARCOⅢ Stage ONFH in young and middle-aged cases involving2 trauma-induced cases,16 hips alcohol-induced cases and 14 hips steriod-induced cases were treated by impacting oral xianling gubao capsule and ligustrazine phosphate tablets combined with bone graft through dislocation of hip joint surgical approach treating ARCOⅢ Stage ONFH in young and middle-aged.Harris score was used to evaluate the joint function,end point events for femoral head collapse more than 4 mm,with femoral head survival as the curative effect evaluation index.Results:All patients had no postoperative infection and thrombosis,one case of postoperative patients with phil total nerve injury symptoms,after treatment,the ankle dorsiflexion muscle strength recovered to grade 3 in 10 months postoperatively,the follow up time was 6—18(averaged 11.8) months,the Harris score was 70.3 + 4.6 at preoperative and 91+6.3 at the last follow up,at the end of the follow-up of femoral head survival rate was 95.5%.Conclusion:TCM combined with Bone Graft through Dislocation of Hip Joint Surgical Approach Treating ARCOITJ Stage ONFH in young and middle-aged was effective in short term and was to be studied further later.更多还原
