添加入双齿围沙蚕的免疫增强型肠内营养制剂对严重烫伤病人的恢复作用

【摘要】 我们研究了添加入沙蚕的早期肠内营养(EEN)对烧伤小鼠的恢复作用。将深Ⅱ度烫伤及对照组小鼠随机分为6组(n=12/组):EN处理组A(TGA,沙蚕蛋白占总蛋白比例为40%w/w),EN处理组B(TGB,30%),EN处理组C(TGC,20%),EN阳性对照组(PCG),空白组(BG)和对照组(CG)。结果表明,TGA组小鼠体重在伤后第14d显著增加(p<0.05),伤后总蛋白和白蛋白含量均比PCG组高,并在伤后第14d显著升高(p<0.05)。白介素-1水平在肠内营养支持组中与CG组相比无显著差异,而受伤小鼠血清肿瘤坏死因子-α水平在伤后第1d则明显高于CG组。此外,EN组内毒素水平在伤后第14d与BG组相比较低,仅TGA组与其具有显著性差异(p<0.05)。在实验组小鼠烫伤部位的全层皮肤组织中,血管内皮因子的合成和释放在任何时相点都比BG组高,而伤后2,7,14d的TGA与BG组之间具有显著性差异(p<0.05)。总之,添加入沙蚕蛋白占总蛋白40%比例的EN制剂能够通过组织重建以及改善免疫、营养等指标来促进伤口恢复。

【Abstract】 Recovery effects of Perinereis aibuhitensis Grube added to early enteral nutrition(EEN) in burned mice was investigated.Deep second degree scaled mice and the control were randomly assigned to six groups(n = 12/group):treatment group A(TGA,40%clamworm protein of total protein w/w),treatment group B(TGB,30%),treatment group C(TGC,20%),positive control group(PCG),blank group(BG) and control group(CG).The results indicated that TGA obviously regained the body weight on post-burn day(PBD) 14(p <0.05).Levels of total protein and albumin in TGA were higher than PCG at each time point,and remarkably increased on the last day of experiment(p <0.05).Interleukin-1 levels in EN groups have no significant difference with CG during PBDs while tumor necrosis factor-α levels from burned mice were significantly increased compared with CG on PBD 1(p < 0.05).Furthermore,endotoxin levels in EN groups downregulated on PBD 14 versus BG,and only TGA descended significantly(p <0.05).The synthesis and release of vascular endothelial growth factor in scald tissue of experimental groups were increased at any time point compared with BG,whereas significant difference was observed between TGA and BG on PBD 2,7 and 14(p<0.05).Overall,EN diet with P.aibuhitensis protein in the ratio of 40%to total protein could promote the wound recovery by improving tissue reconstruction as well as selective immunological and nutritional variables.
