石河子市社区高血压临床路径和管理流程制备

【摘要】 目的:促进社区卫生服务的健康发展,提高社区医疗服务质量,使社区高血压的诊疗、管理和转诊规范操作有序。方法:通过对以往社区卫生服务站及中心临床情况的了解、参考有关文献、临床指南的基础上,制定如下高血压临床路径标准,并进行研究和试行。结果:建立高血压的社区诊疗路径、规范社区医生高血压诊断、治疗、管理和转诊标准,有利于循证医学的最佳证据及临床研究成果推广到高血压控制的最前沿阵地,提高社区医生对高血压的诊疗、管理水平,把握转诊标准。结论:高血压临床路径的实施,可以使高血压患者在社区得到规范和最佳的治疗,具有更好的经济学效益。更多还原

【Abstract】 Objective: To promote healthy development of health services and improve medical service quality in the communities, and to ensure normative procedures for the diagnosis, treatment, management and referral of community hypertensive patients. Methods: By approaching the clinical conditions of the community health service stations and centers and referring to therelevant literature and clinical guidelines, the following clinical pathway and criteria for hypertension were formulated, and were subject to investigation and trial implementation. Results: Thecommunity clinical pathway for hypertension was established, and the the criteria for the diagnosis, treatment, management and referral of hypertensive patients were normalized for the communityphysicians, which was favorable for obtaining the best medical evidences, applying the clinical study outcomings to the front line of hypertension control, improving the community physicians’ ability to diagnose, treat and manage hypertension, and enabling the physicians to understand the referral criteria. Conclusion: Implementing the clinical pathway for hypertension could allowhypertensive patients access to normative and optimal treatment in the communities, thus providing better economic efficiency.更多还原
