健康体检人群血清铁蛋白浓度分布与健康状况的关系探讨

【摘要】 目的:了解杭州市居民的血清铁蛋白(SF)分布,初步解读血清SF与健康状况的关系。方法:回顾性分析2014年在本院参加健康体检的杭州城区居民12090例(有明确的缺铁性贫血患者,孕妇、哺乳和月经期女性除外),把没有既往疾病史和体检结果正常的称为健康组,其他人员为非健康组,再进行性别和年龄分组,对人群血清SF的分布采用秩和检验和卡方检验,做组间和构成比比较分析。结果:血清SF值男性随着年龄增大而降低,男性除健康组中、老年和非健康组青、中年差异不明显,其余年龄纵比、横比差异均有统计学意义(P<0.05)。两组女性均随着年龄增大而血清SF值反而增高,每年龄段都有差异(P<0.05);年龄横比仅中年组差异有统计学意义(P<0.05)。血清SF构成比结果,在<200μg/L,65岁前,健康组:男65.0%,女94.6%,非健康组:男48.5%,女85.5%。65岁后,健康组:男65.0%,女77.7%,非健康组:男60.6%,女76.6%;>600μg/L,男女合计健康和非健康组均为1.3%。结论:血清SF浓度受性别、生理年龄影响,男性随着年龄增大而降低,女性随着年龄增大而增高,男性高于女性,男性非健康人群高于健康组。需重视血清SF增高,可作为体检后的监测指标,有助于健康促进,同时杭州市参考值范围尚需调整。

【Abstract】 Objective:to understand serum ferritin(SF) distribution of the hangzhou residents and to do the preliminary study of relationship between the serum ferritin(SF) and disease.Methods:The clinical data of 12090 hangzhou urban residents who take physical examination in our hosipital in 2014 were analyzed retrospectively(except for patients with iron deficiency anemia and women in pregnant,breastfeeding and menstrual period),people who have no disease history and normal physical examination results were considered as healthy group and other people as unhealthy group,both groups are divide into small groups by gender and age,the distribution of crowd serum SF were analyzed by the rank-sum test and chi-square test,moreover,we do the comparative analysis between the groups and contituent ratio.Results:For male,the serum SF values of men decreased as they get older,except for the middle age group and old group in health group,young group and middle age group in unhealthy group,the difference of the horizontal-to-vertical proportion of other age groups were significant statistically(P < 0.05).For female,The serum SF values of two groups increased with age,the difference between every age groups has statistical significance(P< 0.05);For horizontal comparation,only differences between middle-aged groups were statistically significant(P< 0.05).For constituent ratio results,when the serum SF values of people younger than 65- year- old < 200 μg/L,constituent ratio of male(female) were 65.0%(94.6%) in health group,and 48.5%(85.5%) in unhealthy group.For people older than 65- year- old < 200μg/L,in health group,constituent ratio of male(female) were 65.0%(77.7%),in unhealthy the results were male60.6%/female76.6%;when the SF values were more than 600 μg/L,for male and female,both healthy group and unhealthy group were 1.3%.Conclusion:the serum concentrations of SF was influenced by gender,physiological age,the serum concentrations of SF of men decrease with the increase of age and the women’ s increase with age,The level of male SF was higher than females.For male,the unhealthy group is higher than the healthy group.We should attach importance to high level of serum SF.Taking it as a medical monitoring indicators after health check-up is helpful to health.Moreover,hangzhou reference range of SF is necessary to be adjusted.
