小凸阵与高频线阵双探头联合在新生儿颅脑疾病诊断中的应用

【摘要】 目的探讨小凸阵与高频线阵双探头联合应用在新生儿颅脑疾病诊断中的价值。方法回顾分析了我院2014年5月至2015年4月,108例临床高危新生儿或疑有颅脑疾病超声检查病历资料。其中男68例,女40例,最小10h,最大30d,平均5.6d。结果本组108例,共检出新生儿颅脑疾病40例,阳性率37.03%(40/108)。其中室管膜下出血21例占52.5%(21/40),(双侧室管膜下出血4例;单侧17例,其中合并头皮血肿8例)。未成熟脑5例12.5%(5/40);脉络丛囊肿5例12.5%(5/40);蛛网膜下腔囊肿2例5.0%(2/40);侧脑室增宽2例5.0%(2/40);缺血缺氧性脑病1例2.5%(1/40);侧脑室出血4例10%(4/40)。结论小凸阵与高频线阵双探头联合应用在新生儿颅脑疾病诊断中,优势弥补,清晰地显示颅内细微及浅表结构,明显提高了诊断准确性,有较大的应用价值。

【Abstract】 Objective:To discuss the value of the convex lens probe combined with altotrequent linear array probe in neonatal cerebral disease diagnosis.Methods:A retrospective analysis was made on the ultrasound data of 108 cases of high-risk neonates or suspected cerebral diseases during May 2014 to April 2015 in the hospital.There were 68 male cases and 40 female cases with the average age were 5.6 days old,the youngest was 10 hours and the oldest was 30 days old.Results:40 cases of neonatal cerebral disease were found in the 108 cases.The positive rates were 37.03%(40/108).There were 20 cases of subependymal hemorrhage which accounted for 52.5%(21/40)(4 bilateral cases;17 unilateral cases in which 8 cases combined with scalp hematoma).5 immature brains were accounted for 12.5 %( 5/40) There were 5 cases of choroid plexus cyst in 5.0%(2/40).2 cases of subarachnoid cyst were accounted for 12.5%(5/40).2 cases of fetal cerebral ventriculomegaly were in 5.0%(2/40).1 case of hypoxic ischemic encephalopathy were in 2.5 %( 1/40).And 4 cases of lateral ventricular hemorrhage were 10 %( 4/40).Conclusion The convex lens probe combined with altotrequent linear array probe take complementary advantages in neonatal cerebral disease diagnosis.They clearly showed the intracranial superficial structures which improved the diagnostic accuracy and enjoyed great clinical value.
