MSCT重建技术对鼻骨MOF’s的研究价值
Study in patients with Nasal Bone micro-or occult-fracture by 16-MDCT thin-section reconstruction technology

【摘要】 目的:探讨16层螺旋CT(16-MSCT)对外伤性鼻骨微细或隐匿性骨折(micro-or occult-fracture’s MOF’s)的诊断价值。方法:随访颅面部外伤常规CT检查诊断有鼻骨骨折或可疑骨折者,于复查时采用轴位颅底5mm层厚、5mm层距螺旋扫描后进行0.625mm、1.25mm、2.5mm及5mm高分辨率骨算法重建(简称骨重建),采集数据完整者58例,再到工作站上进行MPR(多平面重建)和VR(容积重建)图像同屏点对点对比观察。按不同重建层厚计算MOF’s的显示分值及显示率,进行自身对照。结果:58例均查出有鼻骨MOF’s,每例1~7处不等。按重建层厚分组比较鼻骨MOF’s分值的显示率:10.625mm层厚的显示率为100%;21.25mm层厚为88.70%;32.5mm层厚为49.52%;45mm层厚为18.27%;经趋势卡方检验分析,差异有统计学意义(Χ2=101.02,P<0.001)。结论:以重建的图像作比较,0.625mm层厚骨重建不用增加辐射剂量就能提高鼻骨MOF’s的显示率;不仅方便、经济,又能为法医学鉴定提供客观、科学、准确的依据;性价比高。
【Abstract】 Purpose:To evaluate the value of 16-MSCT in diagnosis of traumatic nasal bone micro-or-occult-fracture(MOF’s).Methods:Patients with the suspected nasal bone MOF’s were followed,and 58 cases were completed.Conventional 5mm thin-section helical CT scaning with 0.625mm、1.25mm、2.5mm and the 5mm high resolution bone algorithm reconstruction were undergone,in the follow-up exam for these patients.The two-dimensional and three dimensional images were combined to interpret nasal bone MOF’s.The detection rate of MOF’s were calculated and compared for the different thin-section thickness reconstruction.Results:All 58 cases were shown with MOF’s and each case was with 1~7 different locations of MOF’s.The detection rate of MOF’s in different groups was:100% for the 0.625 mm thin-section reconstruction,88.70% for the 1.25 mm,45.52% for the 2.5mm,and 18.27% for the 5mm.The above grouping comparison analyzes.By the tendencyΧ2-test,the differences between the different thickness reconstruction groups were shown with statistical significance(Χ2=101.02,P<0.001).Conclusion:By using 16-MSCT,the thinner the sections are reconstructed,the clearer of MOF’s.will be displayed.It is necessary that for the patients with suspected nasal bone MOF’s,0.625 mm high resolution bone algorithm reconstruction should be used.It is useful to improve the detection rate.
