小骨窗显微手术时机对基底节区高血压性脑出血疗效分析
Effect of the small bone window microscopic operation treatment on treatment effect of patients with hypertensive cerebral hemorrhage in basal ganglia

【摘要】 目的:研究小骨窗显微手术时机对基底节区高血压性脑出血患者治疗效果的影响。方法:将2012年7月-2014年12月期间我院收治的300[300例数据太大,与本院实际相差太大。改为收治高血压性基底节区脑出血患者且最终采用小骨窗显微手术血肿清除术治疗患者140例,后面相关数字均相应改小]例高血压性脑出血患者纳入研究对象,根据手术时机不同分为观察组和对照组各150例,观察组患者接受早期小骨窗显微手术、对照组患者接受延期小骨窗显微手术,比较两组患者的手术情况、术后神经功能及治疗效果。结果:(1)手术情况:观察组患者的手术时间、术后卧床时间、住院总时间、术中出血量、术后引流量均明显低于对照组[(56.32±7.42 vs 88.46±9.45)min,(135.45±17.23 vs 248.34±33.14)ml,(3.42±0.52 vs 5.67±0.78)d,(67.28±9.48 vs 142.64±18.65)ml,(11.35±1.63 vs 19.74±2.34)d](P<0.05);(2)神经功能:术后3个月、6个月、9个月时,观察组患者的CSS评分、NIHSS评分均低于对照组(18.24±2.37 vs 26.34±3.23,15.23±1.85 vs 23.87±3.23,13.45±1.67 vs 20.42±2.89,23.22±3.12 vs 31.42±4.74,20.34±3.84 vs 27.33±4.23,17.31±2.12 vs 23.56±3.84)(P<0.05);(3)治疗效果:观察组良好率明显高于对照组(66.00%vs 51.33%)(P<0.05)。结论:早期小骨窗显微手术有助于减小手术创伤和出血量,加速术后恢复,改善神经功能,提高治疗效果。
【Abstract】 Objective:To study the effect of the small bone window microscopic operation treatment on treatment effect of patients with hypertensive cerebral hemorrhage in basal ganglia.Methods:80 cases patients with hypertensive cerebral hemorrhage in basal ganglia in our hospital from July,2012- December 2014 were enrolled,received small bone window microscopic operation treatment and randomly divided into observation group and control group,observation group received early small bone window microscopic operation treatment,control group received delayed small bone window microscopic operation treatment.Then operation condition and post operation neurological function recovery were compared.Results:(1) Operation condition;observation group operation time,postoperative consciousness recovery time,postoperative bed time,intraoperative bleeding volume,drainage volume were significantly lower than the control group[(56.32 ±7.42 vs 88.46 ±9.45) min,(135.45 ± 17.23 vs 248.34 ±33.14)ml,(3.42 ±0.52 vs 5.67 ±0.78) d,(67.28 ±9.48 vs 142.64 ± 18.65) ml,(11.35 ± 1.63 vs 19.74 ± 2.34) d](P<0.05);(2) nerve function;3 months,6 months,9 months after operation,observation group patients CSS score,NIHSS score were significantly lower than those of control group(18.24 ± 2.37 vs 26.34 ± 3.23,15.23 ± 1.85 vs 23.87 ± 3.23,13.45 ±1.67 vs 20.42 ±2.89,23.22 ±3.12 vs 31.42 ±4.74,20.34 ±3.84 vs 27.33 ±4.23,17.31 ±2.12 vs 23.56 ±3.84)(P<0.05);(3) Treatment effect,observation group good rate was significantly higher than that of control group(66.00%vs 51.33%)(P < 0.05).Conclusion:early small bone window microscopic operation treatment is helpful to reduce operation trauma and bleeding volume,accelerate postoperative recovery,improve neurological function,improve therapeutic effect.
