【摘要】 目的:比较轻比重和重比重小剂量左旋布比卡因复合舒芬太尼蛛网膜下腔阻滞在老年患者经尿道前列腺切除术(TURP)的临床效果。方法:选择蛛网膜下腔阻滞下行TURP的老年患者80例,随机分为4组,每组20例,所有麻醉剂均配制为3ml,详细记录术中数据及变化情况。结果:A组平面达到T10的时间明显短于C组和D组,B组时间明显短于C组(P<0.05)。A组最高感觉阻滞平面明显高于C组,A组感觉阻滞持续时间明显长于其余三组(P<0.05)。A组术后需要镇痛例数明显比B组和C组少(P<0.05),A组和D组术后皮肤瘙痒例数较多(P<0.05);4组术后恶心呕吐发生情况无差异(P>0.05)。4组患者到达最高阻滞平面需要的时间、术中血流动力学变化等无显著差异(P>0.05)。结论:7.5mg重比重左旋布比卡因复合5μg舒芬太尼蛛网膜下腔阻滞应用于老年患者TURP时起效迅速,平面可控性好,运动阻滞轻,术后镇痛效果佳。更多还原

【Abstract】 Objective To compare the clinic effects of hyperbaric and hypobaric levobupivacaine in combination with sufentanil used in elderly patients undergoing transurethral resection of the prostate.Methods 80 elderly patients scheduled for transurethral resection of the prostate were randomly allocated for four groups;the duration of motor block were recorded.The motor block situation of lower limbs,the number of patients who need postoperative analgesia and complications were recorded.Results The time for the sensorial block to reach level T10 in group A were less than group C and D,group B were less than group C( P < 0.05),the block level in group A were higher than group C( P < 0.05).The duration of motor block in group A were obviously longer than others( P< 0.05),the number of need of postoperative analgesia in group A were less than B and C( P < 0.05).The incidence of pruritus in group A and D were more than group B and C( P < 0.05).There were no significant difference among the four groups in the time to reach maximum sensorial block and the intraoperative hemodynamic and postoperative complications( P > 0.05).Conclusion 7.5 mg hyperbaric levobupivacaine combined with 5 μg sufentanil used in spinal anesthesia for TURP in elderly patients provides lighter motor block,better postoperative analgesia and little postoperative complications.更多还原
