尿碘相关评价指标的稳定性及变化(英文)

【摘要】 目的探索不同尿碘相关评价指标的稳定性及其相关变化。方法对来自山东省德州市宁津县和陵县的981名7-13岁儿童进行两次采集24h尿样以及一次性随机尿,记录尿量体积,测定尿碘浓度和尿肌酐浓度。结果在宁津县412名儿童第一次采样的尿量、24h尿碘浓度、晨尿尿碘浓度、24h尿肌酐以及晨尿尿肌酐的中位数分别是640.0ml、192.0μg/L、189.7μg/L、0.56g/L和0.64g/L;第二次尿样采集的尿量、24h尿碘浓度、晨尿尿碘浓度、24h尿肌酐以及晨尿尿肌酐的中位数分别是700ml、171.5μg/L、170.0μg/L、0.59g/L和0.76g/L;两次采样中尿量以及24h尿碘浓度未见统计学差异(P>0.05),第一次采样24h尿肌酐浓度以及晨尿尿肌酐浓度显著低于第二次采样的24h尿肌酐和晨尿尿肌酐浓度(P<0.05),而晨尿尿碘浓度显著高于第二次采样的晨尿尿碘浓度(P=0.034)。对陵县的569名儿童第一次采样的尿量、24h尿碘浓度、晨尿尿碘浓度、24h尿肌酐以及晨尿尿肌酐的中位数分别是700ml、230.2μg/L、203.4μg/L、0.64g/L和0.73g/L;第二次尿样采集的尿量、24h尿碘浓度、晨尿尿碘浓度、24h尿肌酐以及晨尿尿肌酐的中位数分别是810ml、218.3μg/L、244.3μg/L、0.68g/L和0.83g/L;两次采样的24h尿碘浓度和24h尿肌酐未见统计学差异(P>0.05),但是尿量、晨尿尿碘浓度以及晨尿尿肌酐第一次采样结果显著高于第二次采样。结论 24h尿碘浓度相对稳定,而尿量、晨尿尿碘和尿肌酐排出量受气候和饮水的影响,波动较大。

【Abstract】 Background There has been an ongoing discussion on the correctness and suitability on various measurements and methods to estimate iodine excretion.Objective Our study aimed to identify the stability and variations in various measurements for iodine status.Design Two repeated 24-h urine samples and morning spot urine samples were collected in 981 participants aged 7-13 years,enrolled from Ningjin County and Lingxian County.Repeated samplings were implemented in October and November 2013,in Ningjin County and were completed in April and May 2014.24-h urine iodine concentration(24-h UIC),24-h urine creatinine(24-h UCr),morning spot urine iodine concentration(MUIC),morning spot urine creatinine(MUCr) were measured in all urine samples.24-h urine iodine excretion(24-h UEE) was calculated by 24-h UIC multiplying urine volume(Uvol).Results In Ningjin County,there were no statistical differences of 24-h UIC(iM).076),24-h UIE(P=0.563)between two repeated collections in October and November,whereas the UVol,24-h UCr,MUCr and MUIC(P<0.05) were different.In Lingxian County,no statistical differences were observed in 24-h UIC(P=0.793) and 24-h UCr(iM).081) between April and May,while the UVol,24-h UIE,MUIC,MUCr(P<0.05) was relatively higher in May compared to April.The median UIC of total 24-h urine samples and morning spot urine samples were 202.5ug/L and 203.6ug/L,respectively with no significant differences(P=0.545).Conclusion 24-h UIC is a relatively stable and reliable indicator to reflect population iodine status.24-h UIE calculated by repeated 24-h urine collections could be used to evaluate individual iodine intake.
