【摘要】 目的:评价痔上黏膜环形切除钉合术(PPH)治疗环状混合痔的临床疗效。研究方法:将120例患者随机分为PPH组和对照组,各60例,PPH组采用痔上黏膜环形切除钉合术,对照组采用分段弧形切除缝合术,对两种术式疗效、手术时间、住院时间、并发症等进行对比研究。结果:①两组治愈率无显著差异②在手术时间、住院时间、及并发症中精细控便能力、术后疼痛、发生肛门狭窄率、尿潴留、肛门形态等方面,痔上黏膜环形切除钉合术(PPH)组优于分段弧形切除缝合术组。③痔上黏膜环形切除钉合术(PPH)组出现13例吻合口粘膜缩窄,25例患者在吻合器击发时出现下腹部坠痛, 24小时后自然消失。结论:痔上黏膜环形切除钉合术(PPH)具有手术简单、疗程短、并发症少、对肛门功能影响小等优点,近期疗效好,同时也出现了吻合口粘膜缩窄环和下腹坠痛等问题, 部分病例吻合口形成的粘膜缩窄环对术后排便功能的影响,以及可能对远期疗效的影响,尚待进一步观察。更多还原

【Abstract】 Objective TO evaluate the effectiveness of the PPH in the treatment of circular mixed hemorrhoids . Methods 120 patients were divided into two groups with randomized method. 60 cases were treated with the PPH and 60 cases were treated with Whitehead opretion modified . The two groups were compared in therapeutic effects, operation time , hospital stay and complications...etc. Results ①No difference in the curative rate was found between the two guoups. ②the PPH group was better than the modified Whitehead opretion improved group in the aspects of operation time, hospital stay, and fine ability of controlling excrement, postoperative wound pain, incidence of anal abscess, urine retention , the anal appearance etc.③ 13 cases of the PPH group appeared mucosa constriction.of anastomotic stoma and 25 cases appeared intra-operative abdominal pain, after 24 hours abdominal pain was disappear without treating . Conclusion The PPH was a simple, safe procedure with early recovery, few complications, few affection to the anal function and good results in the near future but the PPH also appeared such problems as mucosa constriction of anastomotic stoma and abdominal pain etc. It need further observation that if mucosa constriction of anastomotic stoma will affect postoperative defaecative function and the long-term curative effect.更多还原
