【Abstract】 Human errors are the most influential factors leading to the failure of industrial complex system. This paper expounds the basic analysis procedure and technique, and how to determine radical causes of human errors by E&CF chart in an industrial complex system. It introduces how to conduct screening analysis and what measures taken to correct them. It takes nuclear power plant’s SG valve leakage human error as an example for illustration.更多还原
