尿金属硫蛋白作为职业镉接触生物标志物的研究

【摘要】 [目的] 探讨尿金属硫蛋白作为职业镉接触生物标志物的可行性。[方法] 以我国中南某镉冶炼厂男性作业工人为本研究的职业接触对象,进行接触评定和效应评定,以职业镉摄入、尿镉(UCd)、血镉作为接触指标,尿β2-微球蛋白(UB2M)、尿白蛋白(UALB)、尿N-乙酰-β-D-氨基葡萄糖苷酶(UNAG)作为镉致肾小管和肾小球损害的效应标志物。探讨尿金属硫蛋白(UMT)与上述镉接触和效应标志物间的关系。[结果]UMT含量随着总镉摄入量、血镉和尿镉水平增加而增加,UMT与总镉摄入量、血镉和尿镉的相关系数分别为0.251、0.322和0.437(P<0.05);各效应标志物与UMT间均呈正相关,肾功能异常人群UMT水平明显高于肾功能正常人群。[结论]尿金属硫蛋白含量既反映了职业镉接触水平又与肾损伤效应密切相关。
【Abstract】 [Objective] To explore the usefulness of urinary metallothionein (UMT) as a biomarker in workers exposed to cadmium. [Methods] Urinary cadmium and blood cadmium were used as the exposure biomarkers, urinary β2-microglobulin (UB2M), albumin (UALB) and N-acety-β-D-glucosaminidase (UNAG) were measured as the effect biomarkers of tubular and glomerular dysfunction, respectively. To examine the relationship of urinary metallothionein(UMT) between exposure and cadmimu induced renal dysfunction. [Results] Significant correlations were found between the UMT and total uptake of cadmium, blood cadmium and urinary cadmium, the correlation coefficients were 0.251, 0.322 and 0.437 (P<0.05),respectively. UB2M, UALB and UNAG showed positive correlation with UMT. Cadmium-exposed individuals with renal dysfunction excrete significantly more metallthionein than those with normal renal function. [Conclusion] This study indicated that UMT can be used as biomarker of exposure and biomarker of effect in cadmium-exposed individuals.
