脊柱重度畸形生物力学矫正术(附20例报告)

【摘要】 目的论述脊柱重度畸形矫正的生物力学机制和介绍 M8后路+侧前路手术矫正的方法。方法本组20例重度畸形在手术前后摄全脊柱正侧位片、Bending 片,行 MRI 或 CT 扫描,术后随访1~7年。术后进行对比分析。结果术前 KingⅢ Cobb’s角及后凸成角平均108°,术后平均矫正60°。结论根据重度脊柱畸形矫正的生物力学机制而采用的侧前路 V 形减张松解+后路 M8器械矫正大光明,对严重脊柱畸形,对脊柱、脊髓的安全性好, 有满意的矫正效果。
【Abstract】 Obuective To present the principle of the biomechanics for severe spinal deformity and effectiveness of posterior and side-front M8 instrumentation of clinical application.Methods Patients who underwent operation between September 1997and September 2005 were evaluated by taking,spinal balance X-Ray of the all spine,X-Ray Bending and MRI.Age at surgery aueraged 22.3 years.Followed-up time everaged2.6years.Results The mean preoperative protruded back angle in the King type Ⅲ/Ⅳ was 108degrees,postoperative its mean angle was 60degrees.Conclusion According to the principle of the biomechanise for severe spinal de-formity,their effevt can be satisfactorily obtained using M8 instrumentamom of the posterior and side-front approach.
