陇中牌损伤散预防骨质疏松症的实验研究

【摘要】 骨质疏松症是一种严重威胁中老年人健康的常见病和多发病,但至今尚无理想的防治方法。甘肃省中医院研制的治疗骨折和抗骨质疏松纯中药制剂陇中牌损伤散已临床应用40余年,疗效确切,在患者中享有较高的声誉。但尚缺乏系统的动物实验依据和机理研究。为此,本实验研究了陇中牌损伤散对去卵巢大鼠所导致骨质疏松症的影响。40只SD大鼠随机分成4组,其中3组切除卵巢(OVX组),分别给予口服陇中牌损伤散(OVX+陇中牌损伤散组)、肌注雌二醇(OVX+E2组)和单纯喂食处理(OVX组)。1组进行假手术处理(即进行同样操作,但不切除卵巢),只单纯喂食,用作正常对照(Sham组)。治疗从术后10天开始,持续3个月。实验结束时,采取每一只大鼠的血、尿标本,并摘取其双侧股骨和第一腰椎进行骨密度测量、三点弯曲试验和椎体压缩试验等;血液离心后的血清用于测量碱性磷酸酶(ALP)活性和骨钙素(OC)水平。结果显示,OVX组大鼠的股骨和腰椎骨骨密度显著降低,与Sham组有统计学差异(P<0.05),而OVX+E2组明显抑制了这种变化,与Sham组无统计学差异。OVX+陇中牌损伤散组也阻止了切除卵巢造成的股骨和腰椎骨密度降低,与OVX组之间具有统计学差异(P<0.05),而与OVX+E2组和Sham组无明显差别(P>0.05);OVX组的ALP、OC结果均较OVX+陇中牌损伤散组、OVX+E2组明显增高,具有显著差异(P<0.01);OVX组的股骨三点弯曲实验、腰椎抗压缩的最大载荷、最大应力、能量吸收均降低,与OVX+陇中牌损伤散组、OVX+E2组和Sham组具有统计学差异(P<0.05),但OVX+陇中牌损伤散组与OVX+E2组和Sham组相比,腰椎抗压缩实验中的最大应力、能量吸收指标降低,具有统计学意义(P<0.05)。由以上结果可见,陇中牌损伤散可有效阻止切除卵巢引起的骨代谢高转换状态,防治骨量丢失和骨密度下降,并维持正常的骨骼生物力学特性,说明该药具有理想的预防骨质疏松症疗效。
【Abstract】 Osteoporosis is a common disease that threatens the health of middle-aged and old people greatly. Sunshangsan capsule is prepared by Gansu Provincial Hospital of TCM, and has been clinically used to treat osteoporosis for more than 40 years. However, few studies have been made on its mechanism. In the present study, 40 Sprague-Dawley rats were randomly divided into four groups. Three of them were ovariectomized(OVX) and treated as follows: (1) Sunshangsan was given gastrointestinally (OVX+Sunshangsan);(2) 17β-estradiol was given by subcutaneous injection (OVX +E2); (3)no treatment. The left one group was Sham-operated (Sham) and used as normal control. The treatment started after 10 days of operation and lasted for 3months. At the end of the study, the rats were sacrificed and their sera, urine, femurs, tibias and the first lumbar vertebra (L1) were collected. The left femur and L1 vertebra were first used for BMD measurement, then the left femur underwent a three-point bending test and L1 vertebra underwent the mechanical strength analysis. The serum was used to determine the ALP activity and OC level. As a result, The BMD and mechanical strength of OVX rats were significantly lower than that of Sham rats. The OVX rats had obvious higher ALP activity and OC level than that of Sham rats. Sunshangsan and 17β-estradiol treatment prevent the changes happened in OVX group. The BMD of OVX+Sunshangsan group was lower than that of OVX+E2, but there was no significant difference. Basing on the above results, it can be concluded that Sunshangsan can prevent osteoporosis caused by estrogen deficiency.
