脑干听觉诱发电位与经颅多普勒超声对前庭系统眩晕的诊断价值
Diagnostic value of brainstem auditory evoked potential(BAEP)andtranscranial Doppler(TCD)in patients with vestibular system vertigo

【摘要】 目的:探讨 BAEP 与 TCD 对前庭系统性眩晕的诊断价值。方法:对64例前庭性眩晕者进行了 BAEP 与 TCD 检查, 并与50例正常健康受试者作对照。结果:64例中 BAEP、TCD 异常率分别为56.25%、81.25%,BAEP 表现分为两型:①内耳型 12例:表现为Ⅰ波分化不良或波形消失,PL 延长及其后各波相应延长,LID 相应增大,②脑干型24例:表现Ⅰ波正常,Ⅲ、Ⅴ波分化不良或波形消失,Ⅲ波或Ⅴ波、PL、Ⅰ~Ⅲ及Ⅲ~Ⅴ IPL 延长,ILD 相应增大。TCD 主要表现为椎基底动脉血流速度减低,有或伴有血管张力异常。二者诊断符合率为54.69%。结论:BAEP 有助于前庭系统性眩晕的定位诊断,是鉴别前庭系统周围性与中枢性眩晕比较敏感的方法。TCD 则有助于前庭性眩晕的定性诊断。
【Abstract】 Objective:To explore the diagnostic value of BAEP andTCD in the diagnosis of vestibular system vertigo.Methods:64 pa-tients with vestibular system vertigo and 50 normal controlswere examined with BAEP andTCD instrument.Results:In the patient groups theabnormality rate of BAEP and TCDwere 56.25% and 82.25% respectively.By BAEP,two types of abnormalitywere classified according totheir characteristics:①Inner-ear type(12 cases):showing undifferentiation or disappearance of wave Ⅰ,prolongation of PL of wave Ⅰ and all laterwaves and increase of ILD.②Brainstemtype(24 cases):showing normalwave Ⅰ,undifferentiation or disappearance ofwaveⅢand waveⅤ,prolongation of PL of waveⅢand waveⅤ,also prolongation of IPL of wave Ⅰ ⅢandⅢ,as well as increase of ILD.ByTCD,it mainly showed that the velocity of vertebrobasilar arterywas lower than normal control groups,had or accompanied with the abnormalvessel tone.The coincidence rate of diagnosis betweenTCD and BAEPwas 54.69%.Conclusions:BAEP examination can be used forthe lo-cation of the patient with vestibular system vertigo,and it’s a more sensitive to distinguish between vestibular peripheral and central verdgo.TCD examination will help to find some vascular causes.
