【摘要】 目的探讨脊髓损伤患者泌尿系统感染的预防、治疗及膀胱功能重建的有效方法。方法自1992年在我院治疗的脊髓损伤患者484例,应用一系列治疗方法。包括:1、保持尿道口和会阴部清洁。2、大量饮水。3、导尿(包括间歇引流、间歇性导尿、残余尿量测定、膀胱冲洗等)4、膀胱功能训练。5、电刺激帮助排尿。6、药物治疗(中药、两药)。7、针灸治疗。8、理疗治疗。9、必要时可行泌尿外科手术。结果有39例出现泌尿系统感染,有483例患者除去尿管,有357例残余尿量少于80ml,膀胱功能得到重建,有2例患者行膀胱造口术,1例患者携带尿管至今。结论SCI患者由于排尿障碍和持续性导尿,最容易引起泌尿系统感染,肾盂肾炎,肾盂积水等,进而引起肾功能衰竭。泌尿系统感染发生时,极易导致泌尿系统结石。据统计,导致SCI患者后期死亡的主要原因是泌尿系统感染,因此早期重视泌尿系统感染的预防,利用积极主动的治疗方法,促进膀胱功能的恢复与重建,就能使SCI患者长期存活,达到接近正常人水平。更多还原

【Abstract】 Objectives To discuss the effective means about prevention and cure of urinary system infection, rebuilding the bladder function of the patients with spinal cord injury (SCI) . Methods 484 patients have been received in our study .Some treatments have been adopted, including: 1 .Keeping urethral meatus and perineum clean 2.More drinking 3.Urethral catheterization. (Intermittent effusion; intermittent catheterization; values of residual urine ; doucheing bladder, etc ) 4.Urinary function 5. Electronic stimulation for emiction 6.Medicament 7. Acupuncture 8. Physical therapy 9.Less patients are operated in department of surgery.Result 39 patients suffered from urinary system infection, 483 patients get rid of catheters, values of residual urine of 357 patients are under 80ml, rebuilding of urinary bladder function is effective.2 pacients accept the cystostomy and only one patient need chronically the catheter now. Conclusion Because of urinary dysfunction and durative catheterization, patients with SCI suffer from cystitis, pyelitis, pyelonephritis frequently, and results in kidney dysfunction. Urinary stone often occured when urinary system is infected. Evidence of statistics, urinary system infection is a basilic death causation of patients with SCI.In the forepart, we pay attention to prevention and cure of urinary system infection, take the initiative, and promote rebuilding of bladder function, make patients with SCI live a long time and approach the level of normal person.更多还原
