【摘要】 目的 观察腹式呼吸对亲体肝移植术后呼吸急促的疗效.方法 1例亲体肝移植术后呼吸急促的患儿在呼吸监护下,进行系统的腹式呼吸训练.措施:（1）放松练习:前倾依靠位,头颈部左右轻慢旋转10分钟;（2）腹部加压呼吸;坐位,呼气收腹时,挤压患儿两侧下胸部;吸气时嘱其对抗压力;（3）缓慢呼吸:练习1分钟,呼吸频率每分钟7～12次,休息2分钟后重复;（4）缩嘴样呼气:将小吸管插入水瓶中吹气泡,每分钟7～12次,每次5分钟.以上练习每天1次,每次30分钟.治疗前后记录呼吸频率、进行血气分析.结果 康复介入前,呼吸每分钟55次,轻度紫绀,O₂sat 97%,pH7.411,PO₂18.6kPa,Pco₂5.2kPa,k⁺4.07mmol/L,Hct28%.治疗15天后,自主呼吸明显改善,频率每分钟22次,缺氧症状消失;O₂sat99.7%,pH7.369,Po₂33.6kPa,Pco₂3.9kPa,k⁺4.52mmol/L,Hot33%.结论 腹式呼吸能够明显缓解亲体供肝肝移植术后的呼吸急促,改善缺氧症状.更多还原

【Abstract】 Objective To observe the effects of abdominal respiration on a patient with respiratory rapidity after matched liver transplant. Method Under the respiratory monitoring, a child with respiratory rapidity after matched body liver transplant was trained with abdominal respiration. Measures: (1) Relaxation training: In a relaxed sitting position, let the patient turn head left and right slowly for 10 mins; (2)Breathing under the pressure against abdomen. Sitting, press the patient’s lower thorax while expiring and keep the pressure while inspiring; (3)Slow breathing:Breathing 7- 12times/min for 1 min, and repeat with 2 min interval; (4)Pursed - lip breathing: Blow through a thin tube in a battle of water 7-12 times/min for 5 min. Total training time was 30 min once a day. Breathing frequency and blood-gasanalysis were performed pre and post thearapy. before. Result Before the treatment, the respiratory parameters were frequency 55 times/min,O2sat97% , pH7.411, Po2l8.6kPa, Pco25.2kPa, k+4.07mmol/ L, Hct28%. After 15-day treatment, the respiratory parameters were frequency 22 times/min, AO2sat99.7%,pH7. 369, Po233. 6kPa, Pco23. 9kPa, k24. 52mmol/L, Hct33% . Conclusion Abdominal respiration may improve respiratory function of this patient with respiratory rapidity after matched body liver transplant.更多还原
