膝关节胫骨平台软骨厚度和软骨下骨强度的地形学分布及其相关性分析

【摘要】 目的 研究人膝关节胫至骨平台软骨厚度和软骨下骨强度的分布特点,以及两者之间的相互关系,探讨长期应力刺激下关节软骨和软骨下骨强度的生物力学一功能适应特点.方法 在8例人尸体膝关节的胫骨整个内外侧平台,用网格装置预先标记相互间隔为5mm的测试点,测试关节软骨厚度后,通过特殊的生物力学测试装置对软骨厚度被测点的软骨下骨强度进行测试.运用计算机图像分析软件描述胫骨内外侧平台两被测参数的地形学分布,并分析两参数间的相关性.结果 内外侧平台的软骨厚度从中心区至平台边缘和髁间隆起之间部分呈现向心、逐级递减特征:外侧平台递减幅度高于内侧,内侧平台软骨厚度最大值及平均值低于外侧.软骨下骨强度也显示出从中央区至平台边缘和髁间隆起间的向心、连续递减的分布特征,外侧平台软骨下骨骨强度的最大值区域位于中央偏向背侧,内侧于中央偏向腹侧.且最大值和平均值大于外侧平台.经Statview统计软件分析,胫骨内外侧平台的各相同测试点的软骨厚度和软骨下骨强度呈显著正相关(r分布为0.66,0.63,p<0.01).结论 胫骨平台软骨厚度和软骨下骨强度均呈现规律性分布,内侧平台的负荷大于外侧,表明软骨厚度和软骨下骨强度的分布特点与膝关节的生物力学功能相适应.
【Abstract】 Objectives To investigate the distribution characteristic of cartilage thickness and the subchondral bone strength in human tibial plateau. Method The articular cartilage thickness and the subchondral bone strength were measured from 8 human tibial plateau, topographical variation of two parameters were demonstrated by computerized analyzing program. The correlation coefficient for each specimen were calculated from the paired measurements. Results The cartilage thickness distribution decreased concentrically from the center toward the periphery and the intercondylar eminence in medical and lateral pleateau, The lateral part of the plateau revealed a steeper increase in thickness than the medial part, the cartilage thickness maximum in the medial part of the joint was significantly less than that of the lateral region being therefore considerably above the comparable average value, The distribution of subchondral bone strength also show concentric and continuous charateristic, and, the higher value were found in central ventromedially and dorsolaterlly areas than marginally in the medial and lateral plateau. There was significant correlation between the cartilage thickness and the subchondral bone strength. Conclusion Our results show that the mean loading forces acted on the lateral plateau is less than that of the medial plateau, the cartilage thickness and the subchondral bone strength reveal a basic regularity.
