【摘要】 目的 为了降低平均住院日和减少医疗费用支出,一般全髋关节置换(THA)术后2～3周即出院.出院后绝大部分患者回到家中,由于患者术后早期行动不便,大部分康复部门又尚无开展家庭康复治疗条件,使出院后继续康复较困难.我们曾研究报告住院期间系统康复能提高患者出院时功能水平.在此基础上我们制定了THA出院后以康复教育为主的康复计划,并观察该计划的康复效果和治疗顺应性.方法 50例单侧THA出院后分康复组32例和对照组18例.康复组进行术后6月康复计划,包括1.康复教育:偏制教育手册,包括THA手术基本知识,手术后6月内康复内容和计划,手术后注意事项,手术后可能出现的问题及处理等,文字和标图并用,出院前康复医师作详细的解释.2.术后第2、4、6月康复门诊,康复门诊包括:X线检查,术肢评估、关节活动度和肌力训练方法指导,步态训练,日常生活指导等.3.开通医患电话联系,患者通过电话向医生咨询有关康复问题,医生电话通知患者定期门诊随访.术后六月作FIM,Harris评分及病人满意度评定,满意度采用VAS法,不满1分,非常满意10分.结果 术后6月对照组失访1例,康复组完成三次门诊随访达93.8%.康复组和对照组FIM评分分别是124.11±2.78,122.82±2.93,Harris评分分别是90.53±7.95和85.4±5.86,满意度分别9.21±0.88和8.55±0.92.结论 以康复教育为主的THA出院后康复计划可提高患者的Harris评分及患者的满意度,并且有较高康复治疗顺应性.更多还原

【Abstract】 Objective The compliance and effects of the rehabilitation program after discharge on THA patients were observed. Method 50 THA patients who had underwent two or three weeks intensive rehabilitation were divided into rehabilitation group(N = 32) and control group(N = 18). The post -discharge rehabilitation program include rehabilitation education, outpatient consultant, telephone consultant. FIM, Harris score were determined. Assessment of patient satisfaction was by a visual analogue scale grade from ten(excellent)to one (poor). Results Harris score and patient satisfaction were significantly different in two group. Conclusion The Post - discharge rehabilitation program can improve patients Harris score and patients satisfaction, and has a better compliance.更多还原
