中枢性偏瘫等速测试的研究

【摘要】 目的 探讨肌力训练在中枢性偏瘫患者康复中的必要性和重要性.方法 运用CYBEX-6000型阻力系统对31名病程大于6月能独立行走的中枢性偏瘫患者的两侧膝关节伸肌和屈肌进行肌力标准化测试.结果(1)研究表明患膝屈伸肌的峰力矩在不同的运动速度下(60°/S,120/S,180°/S)均较健侧明显下降,尤以60°/S时峰力矩下降明显(P<0.01).(2)患侧屈肌和伸肌峰力矩出现的时间与健侧相比有提前趋势(P<0.05).(3)虽然患膝的屈肌和伸肌肌力同时下降,但屈肌的肌力较伸肌下降明显,且屈肌、伸肌的峰力矩比值有明显差异(P<0.01).结论 在中枢性偏瘫患者的康复治疗中,在降低肌张力的同时,应加强屈肌和伸肌的肌力训练,尤应加强屈肌的肌力训练.

【Abstract】 Objectives To discuss whether the muscle strength training is necessary to central hemiplegic patients. Method 31 selective central hemiplegic patients who can walk independently and the course is more than 6 months accepted standard isokinetic evaluation on the two sides of knee joint. Results (l)To compare to unaffected knee, the peak torque(PT, Mm)of affected knee in flexion and extension is descend obviously at 60,120, 180/sec speed, especially at 60/sec. (2)The period from the beginning to the peak torque is shorter in the affected knee than that in the unaffected knee(3) Although the muscle strength of flexion and extension is descend in the same time in unaffected knee, flexed muscle strength is decreased obviously, the ratio of flexion/extension muscle strength of two knee is different significantly. Conclusion In the treatment of central hemiplegia patients, we must emphasis on both facts, one is releasing spasticity, another is muscle strength training of the flexion and extension muscle in the affected knee, especially the flexion muscles.
