【摘要】 宫颈癌介入治疗的护理是随着介入治疗学在妇瘤治疗中的广泛应用而产生的,是宫颈癌临床介入治疗的重要组成部分之一,我科从2003年11-2005月11月,共收治接受介入治疗的宫颈癌患者21例,年龄36岁-55岁,中位年龄47 岁,根据FIGO分期法分期,Ⅲ期6例,手术放疗后复发15 例。其中鳞癌12例,腺癌6例,其他3例。使用介入治疗方法:采用Seldinger技术行右侧经皮股动脉穿刺插管,进入双侧髂内前干造影,依造影剂图形观察肿瘤血供,超选择进入靶灶血供支或肿瘤血供支实施灌注和(或)栓塞。常用的药物:鳞癌:丝裂霉素10-14mg,顺铂60—80mg,羟基喜素碱 30mg。腺癌:顺铂60-80mg,喜素30mg,5 U1．5g,溶入 200—300ml生理盐水中,缓慢推注约10—20分钟,后用3—5ml 肝素水冲管。护理分为宫颈癌介入治疗前和宫颈癌介入后护理两大部分。宫颈癌介入治疗前护理包括:心理护理、介入前常规检查的配合、术前患者一般状况的调节、术前床上大小便的训练、皮肤准备及碘过敏试验、标记穿刺部位远端动脉搏动点、胃肠道准备、术前安定和胃复安的给药、患者进介入治疗室前再次测量血压、心率并记录、急救用品和用药的准备等。宫颈癌介入治疗后的护理包括穿刺部位的观察及护理、穿刺侧远端肢体的观察、生命体征监测、臀部皮肤护理、“水化”的护理、胃肠道反应的护理,会阴及阴道出血、排液的观察和护理、病房管理等。以术后护理更为重要。介入治疗的应用,大大提高了晚期及术后、放疗后复发的宫颈癌患者的治疗范围,改善了愈后,有越来越多的妇瘤医生和宫颈癌患者愿意接受和使用介入治疗,使得我们宫颈癌介入治疗的护理得以快速形成和发展。完善的术前准备和指导可减轻患者的心理压力,配合手术的顺利进行;周密的术后护理可减少患者的痛苦,保证患者的顺利康复,尤其是严重并发症的及时观察和处理,往往能起到挽救患者生命至关重要的作用。因此宫颈癌介入治疗护理质量的高低,直接关系着宫颈癌患者的治疗效果和生活质量,为宫颈癌的介入治疗保驾护航。更多还原

【Abstract】 The nursing of cervical carcinoma interventional therapy has emerged from interventional therapeutics extensive applied therapy gynecologic tumors. It one of the important part of the cervical carcinoma clinical interventional therapy .We had received and treated 21 cervical carcinoma patients,which all accepted interventional therapied form November 2003 to November 2005.12 patients are squamocellular carcinoma.6 patients are adenocarcinoma.3 are others.The nursing consist of before interventional therapy nurse and the after interventional therapy nurse.The after interventional therapy nurse is more important As more and more gynecologist and cervical carcinoma patients are willing accept and apply the interventional therapy ,it made our nursing of the cervical carcinoma interventional therapy had rapid emerged and development.Integrity prepare of before operation and guided can lighten patients psychologic burden. It can coordinated surgery successfully carry on.Nurse with the best of care after surgery can lighten the patients pain and can ensure patients successfully restored to health.
