P27、CyclinE和PCNA在上皮性卵巢肿瘤中的表达及临床意义

【摘要】 目的:探讨细胞周期调控因子P27、CyclinE和细胞修复因子PCNA在卵巢癌的发生、发展过程中的重要作用及预后意义。方法:采用间接免疫荧光流式细胞技术和免疫组织化学方法共同检测P27、CyclinE和PCNA在64 例上皮性卵巢肿瘤中的表达情况。其中10例良性,交界性12 例,恶性42例(黏液性15例、浆液性14例、宫内膜样13例) 和正常组织10例作对照。结果:P27在卵巢癌组织中呈低表达(38.9%)、在卵巢良性(80%)或交界性肿瘤(75%)中呈高表达。在卵巢癌组织中的FI值(0.876±0.063)低于良性 (0.978±0.009)或交界性(0.967±0.038)卵巢肿瘤组织, P<0.01。CyclinE和PCNA在卵巢癌组织中呈高表达,在良性和交界性肿瘤中呈低表达,在卵巢癌组织中的FI值高干良性和交界性,P<0.01。上皮性卵巢癌的发生发展与P27表达降低,CyclinE和PCNA 表达升高有关;随着P27表达降低,CyclinE和PCNA 表达升高,上皮性卵巢癌的恶性程度增加,P<0.05。P27、 CyclinE和PCNA的表达与上皮性卵巢癌的临床分期和分化程度有关。P27与CyclinE和PCNA表达呈负相关,R=- 0.564 P<0.01,R=-0.532 P<0.01。CyclinE和PCNA的表达呈正相关R=0.556 P<0.05。免疫组化显示出P27和 CyclinE的亚细胞定位在细胞核且在肿瘤细胞与间质细胞中表达。PCNA的亚细胞定位在细胞核,但只在肿瘤细胞中有表达。在间质细胞中无表达。结论:通过研究P27、CyclinE 和PCNA在上皮卵巢肿瘤中的表达探讨了它们在上皮性卵巢肿瘤发生发展中的作用及相互关系,并为上皮性卵巢癌治疗,检测和预后评估提供了理论依据。

【Abstract】 Objective: To investigate the important function and the prognostic significance about the regulating factor of cell cycle P27,CyclinE and the factor of cytothesis PCNA in the occurrence and development of epithelial ovarian carcinoma. Methods: In this study we examined the expressions of P27,CyclinE and PCNA in 64 Epithelial Ovarian tumor patients with the technology of Flow Cytometry and Immunofluorescence .We compared 10 cases normal tissue with 64 cases including 10 cases benign tumor,12 cases borderline tumor and 42 cases malignant tumor (15 cases mucinous tumor ,14 cases serous tumor,13 cases endometriod tumor). Results: Expression of P27 is low (38.9%) in epithelial ovarian carcinoma, while high in benign (80%) or borderline(75%) epithelial ovarian tumor. FI value of ovarian carcinoma(0.876 ± 0.063) is obviously lower than that of benign (0.978 ± 0.009) and borderline (0.967 ± 0.038) ovarian tumor and P<0.01. Expression of CyclinE and PCNA is high in the epithelial ovarian carcinoma, while low in benign or borderline epithelial ovarian tumor. FI value of ovarian carcinoma is obviously higher than that of benign and bor- derline ovarian tumor and P<0.05.Low expression of P27 and high expressions of CyclinE and PCNA are related to occurrence and development of epithelial ovarian carcinoma, which are positively correlated with the malignant degree, and P<0.05. Expressions of P27, CyclinE and PCNA are correlated with clinical stage and differentiation degree. Expression of P27 is negatively correlated with CyclinE and PCNA r=-0.564 P<0.01, r=-0.532 P<0. 01. The correlation of CyclinE and PCNA expressions is positive r=0.556 P<0.05. Immunohistochemical localization of P27, CyclinE and PCNA in ovarian carcinoma : The subcellular localization of P27 and CyclinE appeared to be nuclear, including tumor cell and stromal cell. The subcellular localization of PCNA is in the nucleus of tumor cell, and it is not in stromal cell. Conclusion: The study has further explored the roles of P27, CyclinE and PCNA expressions and the correlations among them in occurrence and development of epithelial ovarian carcinoma, and provided theoretical basis for monitoring therapy and prognosis assess.
