细胞型FLIPL/S与c-erbB-2在原发性乳腺癌中的表达及意义

【摘要】 目的:细胞型FLIP(cFLIP)是近年来发现的受体凋亡通路抑制因子,在蛋白水平上包括FLIPL和F LIPS 两种类型。研究表明cFLIPL/S在多种肿瘤中表达增多,对肿瘤的发生、发展起到重要作用。本研究通过检测cFLIPL/ S在原发性乳腺癌中的表达情况及其与c—erbB-2表达的相关性,初步探讨cFLIPL/S在乳腺癌发生、发展中的意义。方法:应用免疫组织化学技术检测cFLIPL/S和c—erbB-2 在36例原发性乳腺癌(浸润性导管癌30例,浸润性小叶癌 2例,黏液腺癌2例,髓样癌1,分泌型癌1例)和18例正常乳腺组织(腺纤维瘤旁正常腺体)中的表达水平,分析 cFLIPL/S与c—erbB-2表达的相关性及其与乳腺癌临床生物学特征的关系。结果:(1)cFLIPL/S阳性反应为细胞浆染成黄色或棕黄色,c—erbB-2阳性反应为细胞膜染成黄色或棕黄色。cFLIPL/S在原发性乳腺癌和正常乳腺组织中表达的阳性率和平均染色积分分别为(86.71±2.43)%、6.07 ±0.23和(31.94±2.73)%、2.47±0.31,经半定量分析显示,cFLIPL/S蛋白在乳腺癌中的表达水平显著高于正常乳腺组织(P<0.01)。(2)乳腺癌伴淋巴结转移阳性组 cFLIPL/S阳性表达率显著高于淋巴结转移阴性组(P<0. 05),cFLIPL/s阳性表达率在浸润性癌组织学I-Ⅱ级与组织学Ⅲ级之间差异有显著性(P<0.05)。(3)乳腺癌cFLIPL/ S高表达(平均染色积分≥5)组和低表达(平均染色积分 <5)组中,c—erbB-2阳性率分别为39.41±2.31%和22. 58±1.57%,c—erbB-2阳性率与cFLIPL/S表达水平密切相关(P<0.05)。结论:cFLIPL/S特异性过表达于乳腺癌组织,并可能通过促进c-erbB-2高水平表达参与乳腺癌的发生、发展。

【Abstract】 Objective: Cellular FLIP (cFLIP) is a new inhibitor in receptor- mediated apoptotic pathway and there are FLIPL and FLIPS two forms on protein level. It has been reported that cFLIPL/S is overexpressed in various human carcinoma. We investigated the expression of cFLIPL/S in primary breast cancer and its association with c-erbB-2. Methods: cFLIPL/S and c-erbB-2 were detected by immunohisto- chemistry in 36 primary breast cancer samples including 30 infiltrative ductal carcinomas, 2 infiltrative lobular carcinomas, 2 mucinous adenocarcinomas, 1 medullary carcinomas and 1 secretory carcinomas. It was also determined in 18 cases with normal tissues adjacent to an adenofibroma. The relationship between the expression of cFLIPL/S and c-erbB-2 and clinical characteristics in breast cancer was analyzed. Results: (1) The positive brown-yellow particles of cFLIPL/S were located in the cytoplasm and those of c-erbB-2 in the cell member. The positive rates and average immunostaining scores of cFLIPL/S expression in primary breast cancer and normal breast tissues were (86.71 ±2.43 ) %, 6.07 ±0.23 and (31.94 ±2.73) %. 2.47 ±0.31, respectively. The expression level of cFLIPL/S was significantly higher in carcinoma than in normal tissues (p <0.01). (2) The positive rate of cFLIPL/S was obviously higher in the invasive carcinoma with positive lymph nodes than that with negative lymph nodes (p<0.05). Significant difference also existed between the histological grade 1 to 2 and grade 3 in the invasive carcinoma (p<0.05). (3) The positive rate of c-erbB-2 in cFLIPL/S higher-expressioned group (average unmuno- staining score ≥5) was 39.41 ±2.31 % and that in lower--expressioned group (average immunostaining score <5) was 22.58 ±1.57%. Statistical analysis showed that c-erbB- 2 expression was significantly associated with cFLIPL/S (P<0.05). Conclusion: cFLIPL/ S is overexpressed specifically in breast cancer and it might play a crucial role in the carcinogenesis of breast cancer by promoting c-erbB-2 expression.
