【摘要】 目的:比较两组不同蒽环类药物新辅助化疗方案治疗乳腺癌的疗效及毒性反应。方法:用EC、AC化疗方案治疗Ⅲ期乳腺癌38例,所有患者完成2—4个疗程新辅助化疗后评价疗效。EC方案:表阿霉素(EPI)100 mg／m2 ,d1,静脉注射,环磷酰胺(CTX)500 mg／m2,d1,静脉注射。AC方案:吡柔吡星(THP)60 mg／m2,d1,静脉注射,CTX 600 mg／m2,d1,静脉注射,3周为1个周期。两组化疗前30 min均静脉注恩丹西酮8 mg预防用药。结果:EC组CR 16．7％(3／18),PR 61．1％(11／18),总有效率 77．8％(14／18),AC组CR 10％(2／20),P R 65％(13／20), 总有效率75．0％(15／20),差异无显著性(p>0．05)。两组患者新辅助化疗前患侧腋窝均可触及肿大淋巴结,化疗2个周期后,EC组有12例(66．7％)未触及肿大淋巴结,AC 组有11例(55．0％)未触及肿大淋巴结。两组心脏毒性反应少见,3—4度的血液性毒性EC组比AC组多见,非血液性毒性中除脱发EC组(88．9%)比AC组(75%)多见之外,其余毒性两组发生率相似。(p>0．05)。结论:两组新辅助化疗方案对乳腺癌治疗均有效,毒性反应均可耐受。更多还原

【Abstract】 Objective: To evaluate the efficacy and toxicity of different anthracycline regimen of neoadjuvant chemotherapy in stage III breast cancer. Methods : Thirty-eight patients with stage III breast cancer were treated. All patients received 2 to 4 cycles neoadjuvant chemotherapy. The treatment schedule of EC was consisting of epirubincine 100mg/m2 iv, d1 and CTX 500mg/m2 iv, d1 and AC was consisting of THP 60mg/nr iv, d1 and CTX 600mg/m2 iv, d1, cycles repeated for 21 days. Ondansetron 8mg was injected before chemotherapy in 30 minutes to prevent vomit. Results: The overall response rate of EC group was 77.8% (3 complete response and 11 partial response in 18 patients) and AC group was 75.0% (2 complete response and 13 partial response in 20 patients). There was no significant difference between two groups(P > 0.05). The lymphocyte node could be palpated before chemotherapy in two group, but 12 patients in EC group and 11 patients in AC group couldn’ t be palpated after chemotherapy. There was few cardiac toxicity in two groups. III、IV grade Haematological toxicity in EC group was more than that in AC group. The other toxicity was similar in two group except in EC group(88.9%) was more than that in AC group (p>0.05). Conclusion: Two anthracycline regimen of neoadjuvant chemotherapy has similar effect in stage III breast cancer and their toxicity can be tolerated.更多还原
