【摘要】 目的:探讨环状软骨上次全喉切除术在中晚期喉癌手术中的适应证及疗效。方法:1990年～2000年行环上次全喉切除57例,其中环会厌吻合术37例,环舌吻合术20例。 57例中,男55例,女2例:年龄42—70岁,平均58．8岁。根据1987年UICC标准进行回顾性分期。其中声门型37例, 分别为T2N0M0 5例,T3N0M0 8例,T3N1M0 15例, T3N2M0 7例,T4N2M0 2例;声门上型20例,T2N0M0 3例,T3N0M0 2例,T3N1M0 14例,T4N1M0 1例。喉功能重建主要采用环状软骨舌根或环状软骨会厌吻合术。结果:全部患者恢复了正常吞咽及发音功能,气管套管拔管率为96．5％;5年生存率75．44％(43／57)。结论:该术式适合于中、晚期喉癌患者,在没有降低生存率基础上,患者生存质量得到显著提高。更多还原

【Abstract】 Objective: To discuss the indication and effectiveness of supracrioid subtotal laryngectomy for advanced laryngeal cancers. Methods: 57 cases of advanced laryngeal cancers was operated with supracrioid subtotal laryngectomy, amon them: 37 cases operated with cricohyoidoepiglottopexy (CHEP), the others with cricohyoidopexy (CHP). In these cases, 55 were male and 2 were female with mean age of 58.8 years (range,42 to 70 years). According to the 1987 UICC system, there were 37 glottic cancer(5T2N0M0, 8T3N0M0, 15T3N1M0, 7T3N2M0, 2T4N2M0 ), and 20 supraglottic cancers (3T2N0M0,2T3N0M0,14T3N1M0,1T4N1M0), The new larynx was reconstructed by suturing the cricoid to the base of the tongue and/or epiglottis. Results: The function of swallow and voice were recovered in all cases. 96.5% were decannulated. 5-year survival rate were 75.44% (43/57). Conclusion: Supracrioid subtotal laryngectomy was appropriated for advanced laryngeal cancers, this procedure may improve the quality of life while not reduce the survival rate.更多还原
