【摘要】 目的探讨大肠腺瘤粘膜异型增生诊断的定量判定值,使诊断具有客观性,从而指导其合理的治疗。方法应用图像分析系统对大肠正常粘膜,腺瘤中度异型粘膜及高分化腺癌各20例的腺管的周长（P₁）及所含面积（S₁）,有核带近管腔缘连线的周长（P₂）及所含面积（S₂）以及管腔的周长（P₃）及所含面积（S₃）进行测定．按D=2（S／π）^1／2公式分别算出D₁、D₂、D₃,再计算出P₁／D₁、P₂／ D／2、N／C[（r₁—r₂）／（r₁—r₃）]的值,归纳出其可信区间。结果 NCR、MDYS、CAR三组的P₁／D₁、 P₂／D₂、N／C的可信范围无重叠,且有空档,说明在正常粘膜与中度异型粘膜．中度异型粘膜与高分化腺癌之间存在轻度及重度异型增生。结论应用图像分析仪可以对大肠腺瘤异型增生作出客观化的定量诊断,从而指导其合理治疗。并根据本研究结果,可以设计编写出大肠腺瘤异型增生的定量分析的计算机程序,推广应用于日常的病理诊断之中。更多还原

【Abstract】 Purpose For rational treatment, the diagnostic uqantity value of large bowel adenomas dys plasia were discussed in this paper. Methods With Multimedia pathological Imaging and Word Report System, 20 cases of nor mal mucosa, media dysplasia mucose and high differentiatied adenocarcinoma, respectively, were observed and the circumference of gland duct （P₁） and its area （S₁）, the circumference of the line of nuclei to near the duct edge （P₂）and its area（S₂） and the circumference of duct edge （P₃） and its area （S₃） were measured. According to D=2S/π^1/2, the value of D1. D2. D3 and P₁/D₁, P₂/D₂, N/ C[（r₁—r₂）/（r₁—r₃）]were calculated, respectively, and found beliveable scope. Results The believable scope of P₁/D₁, P₂/D₂ and N/C were not in the same scope in NOR, MDYS and CAR groups. It showed that there were mild and severe dysplasia between normal mucosa and media dysplasia as well as media dysplasia and high differentiated adenocarcinoma. Conclusions With Multimedia pathological Imaging and Word Reprot System, the objective quantity diagnosis could be obtained for dysplasia of large bowel adenomas and rational treatment instructed. According to this results, the computered programs of quantity analysis to dysplasia of large bowel adenomas can be designed and applied it to routine phsiological diagnosis.更多还原
