【摘要】 本文介绍一种直腔癌mil’s术后经会阴部切口行腔内放疗的新方法。本组病人16例均为直肠复发癌二次手术,于术后一周开放由纱布填塞的会阴切口,行腔内放疗,每周1～2次,每次 500CGY,共三次。日后均配合外照射治疗,与同期20例复发癌二次手术后单纯行外照射病例作对照,结果一年内复发率腔内组56％,对照组80％;二年内死亡率腔内组81％,对照组85％;腔内组3 例术前病灶较小者三年内局部无复发。经X~2检验,一年内复发率二组比较P＜0.05差异显著,二年内死亡率两组无显著差异。死亡原因多为远部转移。我们认为此种方法对残留病灶小无远部转移病人有治愈可能,残留病灶大病例有延迟复发作用,但不能控制远部转移,远期疗效尚需进一步分组对照研究。更多还原

【Abstract】 The aim of this paper was to introduce a new method: The intralumen irradiation through perineal incision following resection of rectal carcinoma. When the gauge packed in the perineal incision was taken out about a week after second look, the intralumen radiotherapy was delivered to 16 cases with recurrent rectal carcinoma through the perineal incision. The doses was 500 cGy/time, 1～2 times/week, totally three times. Following the intralumen radiotherapy, these patients all received the external irradiation. At the same time, another 20 cases with recur- rent carcinoma treated only by external irradiation were taken as the control group. The result showed that the one—year recurrence rate for the intralumen irradiation group was 56％ and 80％ for the control group. The two—year death rate for the intralumen irradiation group was 81％ and 85％ for the control group. In the intralumen irradiation group three cases with small lesion before operation had no local recurrence for 3 years after radiotherapy. There was a statistically significant different in one—year recurrence rate (P＜0.05), but no significant fifference in two year death rate (P＞0.05) between the groups. The main cause of death was distant metastases. We think that this method may cure the patients with small residual foci and without distant metastases. And also the intralumen irradiation delayed the recurrence of large residual disease, but it can’t control the distant metastases.
