减压病的加压治疗方法

【摘要】 减压病的主要致病原因是气泡栓塞.加压治疗可以直接消除气泡,是减压病的最根本的治疗措施.我们推荐依据《减压病加压治疗技术要求》(GB/T 17870-1999)选择空气潜水减压病的治疗方案.吸氧有助于机体中氮气的脱饱和,有利于减压病症状和体征的消除。吸氧治疗方案不但疗效较好,治疗时间也较短.有条件应优先选用吸氧治疗方案. 国内时于氮氧潜水减压病和饱和潜水减压病治疗的实践经验不多.氦氧潜水减压病原则上应采用吸氧或呼吸氮氧混合气的治疗方法.轻症或可使用吸氧加压治疗或空气加压治疗,重症必须使用氮氧混合气加压治疗.饱和潜水减压病多发生在减压过程中,可采用暂停减压或适当升高压力(多不超过9m),呼吸治疗用混合气待减压病症状和体征消失或明显缓解后,继续按原方案减压.治疗用混合气的配制可参照《潜水员供气量》(GB18985-2003).

【Abstract】 The recompression therapy is remarkably effective for decompression sickness (DCS). The rational that is developed fit the evidence that the disease is caused by bubbles, and recompression therapy has been presumed to physical reduction of bubble size. We recommend to choose the treatment procedures according to "Technical requirements of recompression therapy for decompression sickness" (GB/T17870-1999) in the treatment of air diving DCS. A review of experimental data and experience with recompression tables is discussed. During the duration of recompression therapy using oxygen is usually successful in treating DCS. Recent evidence suggests that pharmacological effects of hyperbaric oxygen, in addition to the physical effects on bubbles size, gas diffusion and oxygenation, may be important in resolving the disease. Using oxygen may provide an advantage over air treatment in terms of higher success rates and shorter treatment times.We have no so much experience with treating heliox diving DCS and saturation diving DCS. Generally only oxygen or heliox mixture should be used in the therapy of heliox diving DCS. Current recommendations include initial recompression to 0.28MPa, using oxygen treatment procedures. Recompression to 0.4MPa and the use of a mixture of 50% oxygen and 50% helium also offers a good treatment procedure for heliox diving DCS. Patients with pain-only complains tend to can use the same treatment procedures of air diving DCS, those with serious symptoms must use oxygen or heliox mixture in treatment. The occurrence of saturation diving DCS is usually in the duration of decompression, suspension of decompression procedure immediately is necessary. Then rising the pressure of the chamber (less than 9m) and providing the treatment mixture may resolve the symptoms of saturation diving DCS. To composing the treatment mixture, we recommend to refer to "Amount of supply gases for divers" (GB18985-2003).
