颞下颌关节紊乱病的磁共振成像探讨

【摘要】 目的:探讨颞下颌关节紊乱病在磁共振成像上的结构和组织形态学表现。方法:使用PhilipsGyroscan Intera 1.5T超导磁共振成像系统及双侧颞颌关节专用表面线圈,对41例单侧患有颞下颌关节紊乱的患者行双侧82个关节的T2WI、T1WI、T1WI/3D WATS和动态磁共振成像,观察它们的关节结构形态和位置改变情况。结果:可见关节可复性盘前移12侧;不可复性盘前移5侧;关节盘粘连1侧;关节骨结构改变7侧;动态观察活动度过小29侧。结论:磁共振成像能直观地显示颞下颌关节内组织结构的形态和位置,对颞下颌关节紊乱的诊断和治疗提供更直观全面的影像学依据。
【Abstract】 Objective: To discuss the anatomic and morphological changes of the temporomandibular joint’derangement on magnetic resonance imaging system, using T2WI, T1WI, T1WI/3D WATS and MOVIE sequences. Methods: Using Philips Gyroscan Intera 1.5T superconducted magnetic resonance imaging system, 41 patients with temporomandibular joint’ derangement of single side and totally 82 joints were scanned. The changes of positions and morphology of the joints were observed. Results: 12 reversible antedisplacement of joints’cartilage disc and 5 irreversible antedisplacement of joints’ cartilage disc were clearly presented. 1 cases had adhesion of the joints’ cartilage disc. 7 cases had alteration of the articular bone. 29 joints had limitation of articular activity. Conclusion: MRI can displace the position and morphology of the temporomandibular joint directly and thus can provide more image evidences for the diagnosis and therapy of the disease.
